DiscrepenCY: The reference for the dispersion parameter used in the Fuel Handling Accident is not current D**cr4460n: As part of verification of the results of the Fuel Handling Accident (FHA) in the Fuel Building (FSAR Section 15.7.4), ths following calculations were reviewed:

2. UR(B) 228-4," Radiological Consequences of a Fuel Handling Accident in the Fuel Building" Reference 1 is the basis for the current analysis of the FHA in the FSAR. Most of the input data in Reference 1 is taken from Reference 2. In Reference 2 the basis for the atmospheric dispersion parameter (also known as X/Q) used to calculate the offsite dose was referenced to calculation ENVR W156. This calculatinn was requested (M3 RF1-00353). The response to this RFl (M3 IRF 00278) stated that calculation ENVR W156 was never issued and ENVR W221 is the basis for the atmospheric dispersion parameters. Therefore the reference for design input in the current design basis dose calculation is not correct.

(NOTE: A teview of ENVR W221 confirmed that the atmospheric dispersion parameters documented in this calculation are the same as the ones used as input to the dose calculation. Therefore the results of the dose calculation are considered valid and the discrepancy is in the documentation of the input.)

Di*croPency: Auxillary Building Ventilation Exhaust System fan testing acceptance criteria and NCR Disposition Descript6on: The Disposition Details of NCR 392 255 state that during test IST 3 92-028, the air flow through 3HVR*FN68 under dirty flow conditions-did not meet the design criteria of 15,500 Standard Cubic Feet Por Minute (SCFM). The measured flow was 14,835 SCFM. The NCR is dispositioned "Use-As-Is" stating that the '

15,500 SCFM was an estimated " Clean Filter Criteria" and was I developed to provide margin to prevent fan stalling. The  !

disposition details also state that IST 3 92-028 simulated ' Dirty  !

Filter" conditions and the fan did not trip or stall.

PDCR MP# 92103, Rev.1 attached to the NCR identifies the minimum flow point of 15,000 SCFM along with the statement, "The minimum flow point of 15,000 SCFM was establ!shed by test to be the minimum flow capacity of fans 3HVR*FNBA/6B without stalling." There is a contradiction between the acceptance crtteria of IST 3-92-028 and acceptance criteria stated in the PDCR, i.e., the IST uses 15,500 SCFM, and the PDCR identifies 15,000 SCFM.

Diecrepency: Missing guidance in the Containment inspection Procedure.

Descr6pt6on: The containment is to be inspected at least once per refueling interval by parforming a visualinspection of the containment sump, verifying the sumps are not restricted by debris and that there is no evidence of structural distress or abnormal corrosion.

Tech Spec 4.5.2.d.2 requires the containment inspection. The

. Tech Spec requirement is qualitative. SP 3612A.1, Containment inspections, provides the direction for performing the inspection.

The procedure gives examples of loose debris in Attachment 1.

The procedure does not provide similar examples of structural distress or abnormal corrosion. Attematively, the procedure does not specify either minimum training requirements or personal qualifications, a

Discrepancy: Chemistry action limits for RWST not specified.

Description:

The FSAR assumes a value for the lower limit of pH for the RWST water so that the pH of the water recirculated within the containment after an accident is be' ween a pH value of 7.0 and 7.5. Although the plant monitors the pH of the water on a weekly frequency, there are no limits or action statements provided to ensure that the plant meets the FSAR minimum value for pH.

FSAR, page 6.2 41 states "The minimum pH of the spray from the quench spray headers into the containment structure is 4.4.

However, the final pH of the water in the containment structure sump after a DBA, including tha contents of the RWST, is equal to or greater than 7.0 due to the neutralization effects of trisodium phosphate ( TSP ) located in . .*

The water chemistry is monitored weekly, including measuring and recording the pH. However, the chemistry procedure (CP 3802C) and the chemistry data sheet (Chem Form 3802C 1) do not provide either the limit on pH or actions to be taken, if the pH

!3 less than 4.4.

As a consequence, the initial condition for pH may not be met, which could lead to a pH of less than 7.0 for the water recircul&d in the containment following cn accident. The bases for 3/4.5.4 Mfueling Water Storage Tank states ... "This pH band minimizes the effect of chloride and caustic stress corrosion on mechanical systems and components. .. High temperatures and low pH, which could be present after a LOCA, tend to promote SCC. This can lead to the failure of necessary safety systems or components.... Adjusting the pH of the recirculation solution to levels above 7.0 prevents a significant fraction of the dissolved lodine from converting to a volatile form. The higher pH thus decreases the level of airbome lodine in containment and reduces the radiological consequences from containment atmospheric leakage following a LOCA.

Maintaining the solution pH greater than or equal to 7.0 also reduces the occurrence of SSC of austenitic stainless steel components in conhainment. Reducing SCC reduces the probability of failure of components."

Without adequate limits and corrective actions in place, it cannot be assured that the minimum pH as assumed in the FSAR and Tech Spec bases will be met.
