As a result of the inspection, three vlotations were identified. The NRC is concerned

because the violations appear to have resulted from a lack of attention to detall. The

violations are cited in the enclosed Notice of Violation (Notice) and tha circumstances

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and
