NOTICE OF VIOLATION AND INSPECTION REPORT NO. 50-263/98002(DRP)
FOR MONTICELLO NUCLEAR POWER PLANT
Dear Mr. Wadley:

On February 23,1998, the NRC completed an inspection at the Monticello reactor facility. The
enclosed report presents the results of that inspection.
During the six-week period covered by this inspection, the conduct of activities at the Monticello
facility was acceptable. However, several violations of NRC requirements were identified. Three
of the violations cited correspond to personnel failing to follow procedures or failing to provide
adequate instructions to maintenance and operations personnel. These violations demonstrated
a lack of attention-to-detail during work activities.
The fourth violation was cited for an inadequate Technical Specification (TS)-required
surveillance test which contained incorrect acceptance criteria. This violation appeared to be
due to procedures, affected by a TS change effective in October 1995, that were not updated to

reflect the change in a timely manner. The inspectors also identified weaknesses in the
TS Amendment process, such that other effective TS changes may not be reflected in other

procedures. In your response to this violation, please describe your corrective actions to address
this concern.

These violations are cited in the enclosed Notice of Violation (Notice). The circumstances

surrounding the violations are described in detailin the enclosed report. Please note that you are
required to respond to this letter and should follow the instructions specified in the enclosed
Notice when preparing your response. The NRC will use your response, in part, to determine
whether further enforcement action is necessary to ensure compliance with regulatory
requirements.
