ENGINEERING EVALVATION REPORT - LOSS OF RESIDUAL HEAT REMOVAL SYSTEM EVENT AT PILGRIM NUCLEAR POWER STATION .. DECEMBER 21, 1981

_ Enclosed is my evaluation of a loss of residual heat removal (RHR) cooling

' event which occurred at Pilgrim on December 21, 1981. In this instance RHR cooling flow was lost while a system pump was operating. This condition went unrecognized for almost five hours. The consequences were minimal sicce the plant was in the very late stages of an extended refuel,ing outage.

I believe the most important feature of this event was the failure of the operators in two shifts to recognize this loss is serious. However, even when considering this fact, the overall event does not appear to meet the criteria necessary to classify this event as an abnormal occurrence. It does seem,' though, to be a good candidate for " Power Reactor Events."

Also, it serves to reinforce the recommendations put forth in IE Circular 81-11 " Inadequate Decay Heat Removal During Reactor Shutdown" '(issued July 24,1981.) . .
