NRC INTEGRATED INSPECTION REPORT 50-327/98-03 AND 50-328/98-03 AND
. Dear Mr. Zeringue:
This refers to the inspection conducted on February 1 through March 14. 1998,
at your Sequoyah facility. The enclosed report presents the results of that
inspection.
During the inspection period your cc,1 duct of activities at the Sequoyah
facility was generally characterized by good operations. However; negative
findings were identified-for a deficient surveillance procedure and a lack of
guidance for reviewing the printout frc.,m the control room alarm printer.
In
addition.~further review is necessary to resolve issues with waste gas samples
that indicated high oxygen concentration in the pressurizer relief tank.
Maintenance review of issues was noted to be good.
Engineering support was
considered to be good, however; further review is necessary to resolve issues
related to valve stroke testing.
Plant support was considered to be good with
an exception that a problem evaluation report and a work request were not
initiated as required.
Based on the results of this inspection, the NRC has determined that a
violation of NRC requirenents occurred.
The violation is cited in the
enclosed Notice of Violation (NOV) and the circumstances surrounding it are
described in ' detail in the subject inspection report. The violation involved
failures to initiate a work request when waste gas samples could"not be
obtained and failure to initiate a problem evaluation report. Both examples
.
were related to sampling of the pressurizer relief tank and initial samples
that indicated high oxygen concentrations in the tank.
You are ' required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
The NRC will
use your response. in part, to determine whether further enforcement action is~
necessary to ensure compliance with regulatory requirements.
