SUBJECT: NRC INSPECTION REPORT 50 313/97 14; 50 308/97 14

Dear Mr. Hutchinson:

An NRC inspection of Units 1 and 2 steam generator activities was conducted onsite

during May 27 through June 20,1997, at your Arkansas Nuclear One, Units 1 and 2  :

reactor f acilities, with additional offsite review performed during July 14 through August 7, ,

'

1997. The offsite inspection included review of a position paper provided by your staff

regarding the 1990 bobbin coil examination indications obtained during Unit 2

Outage 2F901, that were not identified as missed indications until Refueling Outage 2R12

in 1997. The position paper has been included as Attachment 2 to tho inspection report.  ;

The enclosed report presents the scope and results of this inspection. A telephone exit

e interview was conducted on August 8,1997.

being considered for escalated enforcement action in accordance with the " General

Statement of Policy and Procedure for NRC Enforcement Actions" (Enforcement Policy),

bobbin coil sizing qualification criteria; (2) the f ailure to remove Unit 2 tubes from service

'

during Outage 2F901, which contained flaws that exceeded the plugging litnit of the

Technical Specifications; and (3) the lack of prompt corrective action in November 1995

prior to returning potentially defective sleeved tubes to service.

While we recognize that the eddy current program enhancements implemented during

Refueling Outage 2R12 were instrumental in the identification of the existence of bobbin '

_c' oil small voltage flaws in prios oJtage data, we remain concerned about the circumstances

_ pertaining to the change in status, for a significant number of Unit 2 tubes, to one. requiring-

plugging or repair af ter only approximately 5 months of unit operation from Outage 2F901.

Also of concern is the adequacy of management controls over the adoption and

implernentation of a now sizing technique for depth of intergranular attack in Unit I steam

generator tubing, in particular, wo would like you to discuss the specific details involved in

your decision to use tubet data obtained from another licensee's steam generators to ,

'

implement this technique. The acceptanco in 1995 of contractor proposed "use as.is"

dispositions for oddy current examirvion identified wold anomalies in installed Unit 2

slooves is also of concern, in that no attempt was made to understand the nature of the

anomalios. Eddy current examination identiflod wold anomalios in gas tungston arc wolded

slooves woro subsequently found by another licensoo to be indicators of actual wold defect

conditions.

A prodocisional enforcement conference to discuss the apparent violations has been

'

scheduled for September 20,1997, at 12 noon in the Hogion IV offico. The decision to

. hold a prodocisional enforcement conferenco does not mean that the NRC has mado a final

determination that violations havo occurred or that enforcement action will be taken. This

conference is being hold to obtain information to enable the NRC to make an enforcement

decision, such as a common understanding of the f acts, root causes, missed opportunities

to identify the apparent violations sooner, corrective actions, significance of the issues and

the need for lasting and offectivo correctivo action, in addition, this is an opportunity for

you to point out any errors in our inspection report and for you to provido any information

concerning your perspectivos on 1) the severity of the apparent violations, 2) the

application of the f actors that the NRC considers when it datormines the amount of a civil

penalty that may be assessed in accordance with Section VI.B.2 of the Enforcement Policy,

and 3) any cther application of the Enforcement Policy to this case, including the exorciso

of discretion in accordanco with Section Vll. This conference will be open to public

observation in accordance with the Enforcement Policy.

You will be advised by separato correspondence of the results of our deliberations on this

matter. No response regarding the apparent violations is required at this timo,

in accordanco with 10 CFR 2.700 of the NRC's " Rules of Practice," a copy of this letter

and its enclosure will be placed in the NRC Public Document Room (PDR),
