NRC INSPECTION REPORTS 50-456/98003(DRS); 50-457/98003(DRS)

Dear Mr. Kingsley:

On January 30,1998, the NRC completed a special inspection at your Braidwood Nuclear

Power Station Units 1 and 2. The purpose of this inspection was to review activities relative to

the Unit 2 reactor trip on January 26,1998. The inspection included an assessment of

maintenance activities on the turbine digital electro-hydraulic control system that initiated the

reactor trip.

The inspection identified deficiencies in the maintenance activities on the Unit 2 turbine digital

electro-hydraulic control system including: 1) use of a blanket work request that bypassed the

normal work control process,2) use of uncontrolled documentation to perform troubleshooting

on the nuclear programmable logic circuit cards, 3) failure to follow vendor instructions for

configuring the nuclear programmable logic card that caused the reactor trip,4) failure to use a

procedure for bench testing the nuclear programmable logic card, and 5) failure to assign the .

correct priority to emergent work that resulted in bypassing normal approved work controls.

Altho' ugh the components involved are balance-of-plant components, we are concemed that

similar deficiencies in operational analysis department work control activities may carry over

into activities involving safety related components.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice" a copy of this letter and its

- enclosures will be placed in the NRC Public Document Room.-
