SUBJECT: NRC INSPECTION REPORT 50-298/98-12

Dear Mr. Horn:

From March 16-19,1998, a routine, announced emergency preparedness inspection was

i

conducted at your Cooper Nuclear Station reactor facility. The enclosed report presents the

I

scope and results of that inspection.

One exercise weakness was identified during the simulator walkthroughs, as discussed in

Section P4 of the enclosed inspection report. We are concemed because the weakness

involved the failure of one crew to demonstrate the ability to effectively implement key elements

of the emergency plan. The level of performance indicated a need for remedial actions. We

understand that remedial training for the crew was conducted immediately following the

inspection and that a root cause analysis was initiated to determine the reason for the failure.

l

Exercise weaknesses, according to 10 CFR Part 50, Appendix E.IV.F.2.g, are inspection

findings that need to be corrected by the licensee. Therefore, you are requested to provide this

l

office with a description of your corrective measures and your schedule for completing these

i

actions within 30 days after receipt of this letter. Your response should also include the results

of the root cause analysis and your actions to ensure that other crew and emergency rt ponse

I

organization members understand their roles and responsibilities. This weakness is of podicular

concem because previous training cycles failed to identify problems with perfortnance and

potentially misleading expectations with regard to shift emergency response duties.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its

enclosure will be placed in the NRC Public Document Room.
