NRC NOTICE OF VIOLATION AND INSPECTION REPORT

NO. 50-282/96003(DRP); 50-306/g8003(DRP) FOR PRAIRIE ISLAND

NUCLEAR GENERATING PLANT

Dear Mr. Wadley:

On February 24,1998, the NRC completed an inspection at your Prairie Island Nuclear

Generating Plant. The enclosed report presents the results of that inspection.

l

During the six-week period covered by this inspection, continued improvements were noted in

procedure adherence by operators, in addition, operators exhibited improvements in control

.

room access management, communications, and professionalism. A reactor coolant system

leak on Unit 2 was rapidly identified and conservative action was taken to shut down for repairs.

'

The shutdown and outage operations activities were conducted well. Although compensatory

.

actions for additional leak monitoring on Unit 1 were slow to be established, once started, they

were comprehensive. Removal, nondestructive examination, and repair activities associated

l

with the Unit 2 leak from the partial length control rod drive mechanism were managed by a

i

station task force and were conducted well. Effective radiation control practices were used to

l

maintain reasonably low doses during the project. Also, during the inspection period, the backlog

l

of maintenance work orders, control room deficiencies, and temporary modifications were very

low indicating a strong maintenance program which was receiving suitable management

attention.

Prompted by NRC concems, your staff conducted special control room ventilation testing, and as

a result, determined that in-leakage of both steam and airbome radioactivity during accidents

could have been greater than design assumptions, indicating a need for additional work to

resolve the issues. In addition, an inadequate review of breaker protection logic by a system

engineer led to an inappropriately sequenced work order and resulted in an engineered safety

feature system actuation and a violation of NRC sequirements. This was the fourth documented

case in a little over a year in which an inadequr.te review of logic circuits led to problems during

maintenance or testing activities.

The violation identified above is cited in the ernlosed Notice of Violation (Notice), and the

circumstances surrounding the violation are dercribed in detailin the enclosed report. Please
