50-265/97011(DRP), AND NOTICE OF VIOLATION

Dear Mr. Kraft:

This refers to the inspection conducted on June 17 through July 28,1997, at the

.

Quad Cities Nuclear Power Station. 'The purpose of the inspection was to determine

'whether activities authorized by the license were conducted safely and in accordance with

NRC requirements. During this period routine resident inspections were performed. Based

,

on the inspectors' findings, performance overall at the Quad Cities station was

-

- characterized by safe and conservative operations.: At the conclusion of the inspection,

j

the findings were discussed with those members of your staff identified in the enclosed

I

report,

i

This report documents three activities that the inspectors observed to be well planned and

controlled. ' Operators generally performed well during the Unit 2 reactor startup. Work

planning for the safe _ shutdown makeup pump system maintenance outage was thorough,

resulting in the resolution of several system deficiencies. Coordination of work groups in

response to the identification of a Unit 2 fuel leak allowed for the effective and timely

Identification of the 'affacted fuel bundle.

During the inspection period, three violations were identified. One violation pertained to

design control and consisted of two examples where your staff failed to adequately-

incorporate important design information into plant operating procedures. The other two

violations pertained to the failure to adhere to. Technical Specification (TS) requirements.

Specifically your staff failed to verify the positions of _ valves in the residual heat removal

service water system and the failed to lock or attend a posted locked high radiation area

- door,

in previous correspondence, the NRC commented' on the momentum you have been able to

-- achieve in improving performance in the area of plant operations. However, the NRC is

concemed that this momentum is no longer being maintained. This conclusion is based on

f

- recent inspection findings and those identified during this inspection period that include:

inappropriate entry into shutdown limiting conditions for operation to perform planned

!

. maintenance; improper use of an annunciator alarm to initiate a change in a pressure

_

control setpoint; not investigating a frequently alarming core spray annunciator for several

' days; and the failure to identify a significant increase in offgas activity for two shifts.
