SUBJECT: LICENSE AMENDMENT

Please find enclosed License No. 50-19913-01. You should review this license carefully and be

sure that you understand all conditions. If you have any questions, you may contact the

reviewer who signed your license at (925) 975-0250.

The NRC staff has rr, viewed your sealed source leak test results and your final surveys of the

former hot laboratory and imaging room. Based on its review, the staff has concluded that all

licensable radioactive material has been removed from these port;ons of your facility and that

residual radioactive material in these areas attributable to licensed material does not exceed

current NRC criteria.

,

Based on these conclusions, no further remediation is required. The former hot laboratory and

imaging room are suitable for unrestricted release and have been deleted from the license as

authorized use locations.

NRC expects licensees to conduct their programs with meticulous attention to detail and a high

standard of compliance. Because of the serious consequences to employees and the public

which can result from failure to comply with NRC requirements, you must conduct your program

involving radioactive materials in accordance with the conditions of your NRC license,

representations made 'n your license application, and NRC regulations. In particular, note that

you must:

Operate in accordance with NRC regulations 10 CFR Part 19, " Notices, Instructions and

Reports to Workers: Inspection and Investigations," 10 CFR Part 20," Standards for

Protection Against Radiation," and other applicable regulations.

Possess radioactive material only in the quantity and form indicated in your license.

3.

Use radioactive material only for the purpose (s) indicated in your license.

4.

Notify NRC in writing of any change in mailing address (no fee required if the location of

radioactive material remains the same).

40.420)(2),70.380)(2), and 72.54 (t)(2). This includes submitting data in the following

units: gamma radiation in units of mSv/br (mR/hr) at one meter from surfaces,

radioactivity units of MBq/100cm2 (dpm/100cm2)(removable and fixed) for surfaces

MBq/ml(mci /ml) for water and Bq/g (pCilg) for soils and concrete. See attachments 1

Area wipes and surveys were performed piior to transfer of radioactive material from the hot lab.

All the results were within normal limits. All lead shielding was transferred to a portable cart.

The current, generator, QC sources, and radioactive materials waste container were placed in the

lead shielding on this cait.100% area survey was performed of the countertops, shelving, and

j

waste storage area prior to the transfer to the new hot lab. See enclosed attachment A for results.

Then a 100% asca survey was performed of the floor, walls, ceiling, and door of the hot lab. All

surveys were .02mR/hr or less. Using the bicron surveyor 2000 with pancake probe with serial

number B340D. Last cal date of 1/21/98 with lowest detectable level of confidence of.02

The wipe test were performed using the atomic products nucleus well counter serial number

materials were transferred into the new hot lab the cart was surveyed and wipe tested with the

6. The sun ey instrumentation used for the final survey along with the certification that

each instrument has been properly calibrated and tested and the minimum detectable

activity (MDA) for each instrument. This information is needed for instruments used

for measuring exposure rates and for those used for analysis of wipes, soil and water

samples,etc. (See attachment 3)

until all waste (and other licensed materials / sources) have been removed from the site.

survey data, then these areas will have to be surveyed following removal of the waste

and the data submitted and reviewed before an onsite Inspection and/or license

termination.
