SUBJECT: NRC INSPECTION REPORT 50 361/97 17:50 362/97 17 AND NOTICE OF
VIOLATION
Dear Mr. Ray:
An NRC inspection was conducted July 6 through August 16,1997, at your San Onofre
Nuclear Generating Station, Units 2 and 3 reactor facility. The enclosed report presents
the scope and results of that i,spection.
During the 6-week period covered by this inspection, your conduct of activities at
San Onofre was generally characterized by safety conscious operations, sound engineering
and maintenance practices, and careful radiological work controls. This was especially
significant given the challenges posed by the dual unit outage. However, weaknesses in
operator attention to detail, knowledge of plant conditions, procedural adherence, the
communication of management expectations, and the overall quality of operational
procedures were noted. Your continued attention is warranted.
in addition, baseo on the results of this inspection, the NRC has determined that four
violations of NRC requirements occurred. These violations are cited in the enclosed Notice
of Violation (Notice) and the circumstances surrounding the violations are described in
detail in the enclosed report. Please note that, for Violations A, B, and D, you are required
to respond to this letter and should follow the instructions specified in the enclosed Notice
when preparing your response. The NRC will use your response, in part, to determine
whether further enforcement action is necessary to ensure compliance vf.th regulatory
requirements.
The NRC has concluded that information regarding the reason for Violation C, the

corrective actions taken ano those planned to correct and prevent its recurrence are

already adequately addressed in Enclosure 2. Therefore, you are not required to respond to' /
this letter for Violation C unless the description therein does not accurately reflect your
corrective actions or your position, in that case, or if you choose to provide additional
information, you should follow the instructions specified in the enclosed Notice,
in accordance with 10 CFR Part 2.790 of the NRC's " Rules of Practice," a copy of this
letter, its enclosures, and your response will be placed in the NRC Public Document Room
(PDRh To the extent possible, your response should not include any personal privacy,
proprietary, or saf] guards information so that it can be placed in che PDR without
redaction.
