SUUJECT: NRC INSPECTION REPORT 50 298/97 07 AND NOTICE OF VIOLATION

Dear Mr. Horn:
l
'
An NRC inspection was conducted June 29 through August 9,1997, at your Cooper
Nuclear Station reactor facility. The enclosed report presents the scope and results of that
inspection.
l
During this inspection, a focused review of operability evaluations and 10 CFR 50.59
evaluations was undertaken. Operability assessments often did not consider relevant
design information or did not document consideration of the relevant design information.
Actions were initiated to address this weakness.10 CFR 50.59 safety evaluations were
found to be generally complete; however, several f ailures to identify, correct, determine
the cause, or prevent recurrence of conditions adverse to quality were identified, in view
of previously documented weak operability assessments and corrective actions, we are
concerned that the corrective actions you have. implemented to improve these areas of
performance have not been more effective.
Also, during this inspection, a violation of NRC requirements was identified conceming the
ineffective implementation of design requirements in plant procedures. This violation is
cited in the enclosed Notice of Violation (Notice) and the circumstances surrounding the
violation are described in detailin the enclosed report.
Please note that you are required to respond to this letter and should follow the
instructions specified in the enclosed Notice when preparing your response. The NRC will
use your response, in part, to determine whether further enforcement action is necessary
to ensure compliance with regulatory requirements,

its enclosure (s), and your response will be placed in the NRC Public Document
Room (PDR). To the extent possible, your response should not include any personal
