NRC INSPECTION REPORT NO. 50-263/98004(DRP) FOR MONTICELLO

Dear Mr. Wadley:

On April 14,1998, the NRC completed an inspection at the Monticello reactor facility. The

enclosed report presents the results of that inspection. Also enclosed is a copy of the Office of

Nuclear Reactor Regulation (NRR) response, dated February 11,1997, to an NRC Region ill

Task Interface Agreement regarding acceptability of use of leak-before-break analysis for

environmental qualification of equipment in some operability evaluations at Monticelle. This

issue is discussed in Section E 8.2 of the enclosed inspection report.

Dudng the seven-week period covered by this inspection report, the conduct of activities at the

Monticello facility was generally characterized by safety-conscious operations and careful

radiological work controls. The preparation work performed to support the 1998 refueling outage

was effective as evidenced by the excellent coordination and execution of work to date.

However, the inspectors identified some specific weaknesses with the performance of the

engineering and maintenance staff during this inspection period. One weakness involved the

modification process in that the installation of a jumper / bypass to support replacement of a

condensate service water system relief valve was not considered in the associated

10 CFR 50.59 review for this modification. Another weakness pertained to the thoroughness of

the evaluation of a water hammer event associated with the high pressure coolant injection

system. Although the inspectors did not identify any violations of regulatory requirements in

connection with these activities, we are concemed that your staff did not aggressively pursue

these issues until prompted by NRC inspectors. We will continue to monitor the affectiveness of

your staff's assessment of these issues.
