l SUBJECT: INSPECTION REPORT NO. 5(L456/98002(DRP); 50-457/98002(CRP) AND

                             NOTICE OF VIOLATION

              Dear Mr. Kingsley-
            On March 9,1998,' the NRC completed an inspection at your Braidwood Units i and 2 reactor
            facilities. The enclosed report presents the results of that inspection. During the six-week penod
            covered by this inspection, your conduct of activities was genere'ly characterized by safety-

            conscious operations, sound engineering and maintenance practices, and careful radiological
           work controls.

           We are concemed about two failures to follow station administrative procedures.
          'Two violations associated with these failures are described in the enclosed Notice of Violation
           (Notice). The first violation involves the failure to perform an independent verification of proper
           system alignment following the repositioning of safety related valves as required by station
           administrative procedures. The second violation involves the failure of the fire watch to patrol
           the Unit i lower cable spreading room every 15 minutes as required by station administrative
           procedures. These violations were inspector-identified. The failure to follow procedures has
           been highlighted in previous inspection reports and appears to be a remarring problem.
          'We are also concemed about a failure to co spb with Technical Specification requirements.
          Operators failed to take the actions requirst of feAnical Specifications following a change in
           Unit 2 operational modes with both trains of the boron dilution protection system inoperable.
          Since this issue was non-repetitive, self-identified, and corrected, it was considered a non-cited
          violation.
          Regarding the violations described in the enclosed Notice, relating to the failure to follow
          procedures, please note that you am required to respond to this letter and should follow the

l ' instruchons specified in the enclosed Notice when preparing your response. Your response

          should not only detail the actions you have taken or plan to take for the specific issues identified,
          but should also be more broadly focused and address what actions you plan or have taken to
          ensure that your corrective action process is effective. The NRC will use your respens'), in part,
