AE0D CASE' STUDY REPORT:' BREACHING OF THE ENCAPSULATION OF SEALED WELL LOGGING SOURCES Enclosed for your information is the AEOD case study report, " Breaching of the Encapsulation of Sealed Well Logging Sources." This report' initially issued for peer review on March 7,1984, has been revised based on the comments received during the peer review period. This revision resulted in some minor changes in the content of the report. However, the findings, conclusions, ~~ and recommendations are essentially the same as presented in our preliminary case study report. The recommendations in our report have been directed to NRC actions toward our licensees. We believe, however, that SP should consider Recommendation 5, "The interfaces between the well owner, well logging company, and companies specializing in recovery operations, should be defined in order to establish the regulatory responsibility and authority over. recovery operations involving sealed sources." We believe that Section 39.15 of the proposed Part 39 that requires a written agreement between the NRC licensee.and the well owner and operator is responsive to our recommendation 5. We note that Part W, Radiation Safety l Requirements for Wireline Service Operations and Subsurface Tracer Studies (Suggested State Regulations for Control of Radiation,' 1982), does not require l a written agreement of the type defined in Section 39.15. We recommend that I SP request that the Conference of Radiation Control Program Virectors Inc., consider incorporating a requirement similar to the written agreement defined in'- 1 Section 39.15 in Part W when the next revision is being'considerci. fCPlb b 8611060178 861031 PDR FDEA

Please be advised that I certify that the following persons have been trained by Applied Health Physics, Inc. and myself, that they have satis-factorily completed that training and passed an apprcpriate examination, and that they have such additional operational experience as is required for them to be qualified engineers. 1. Mcward T. Reed II 2. Harold V. Reynolds We would like, at this time, to have these two' names added to our original list appearing in our letter dated Octob~er 28, 1983. We also have three Electronic Technicians who, at times, will be ( handling the sources in order to determine whether our down hole instruments are working properly. The time involved in this operation will be approximately 5 minutes and will be performed on the average of 6 times per week. These people will be wearing their film badges at all times. Also, surveys will be made and recorded to determine the mr/h they will be exposed to. The persons involved have also passed the examination given by Applied Health Physics. Their names are: l l 1.
