Dear Mr. Smith:

This is in further response to your letter dated July 3, 1986, in which you requested, pursuant to the Freedom of Information Act (F0IA), specified records concerning the breaching of the encapsulation of sealed well logging sources.

The additional records listed on the_ enclosed Appendix A (continued), and the released portions of records listed on.the enciased Appendix _B are being made available for public inspection and copying at the NRC Public Document Room (PDR), 1717 H Street, NW, Washington, DC 20555, in PDR file F01A-86-501 under your name.

The staff also identified the 1982 Edition of the _ Suggested State Regulations for Control of Radiation, Volume I, Ionizing Radiation prepared by the Conference of Radiation Control Program Directors, Inc., the NRC, EPA, and HHS, which is available from the National Technical Information Service (NTIS) Department of Commerce, 5285 Port Royal' Road, Springfield, VA 22161. The identification number for this document is PB83252569, and the price is $34.95 plus $3.00 handling fee. This document is~out of print and is not available from the Government Printing Office.

Portions of records II-B1, IV-81, and VI-B1 through VI-B36 listed on Appendix B have been deleted in order to withhold the names, home addresses and home telephone numbers of individuals. Because disclosure of this information would

, constitute a clearly unwarranted invasion of personal privacy, it is being l withheld from public disclosure pursuant to Exemption (6) of the F0IA (5 U.S.C.

552(b)(6)) and 10 CFR 9.5(a)(6) of the Commission's regulations.

The case study was commenced in late 1983 and refers specifically to the following companies and incident dates: Consolidated Coal Company - August 19, 1982; Dresser Atlas - November 25, 1982; Professional Logging Services ,
