I&C Department investigation of this event revealed that a cable between the detector and the monitor electronics was crimped which created a sporadic open circuit. The means by which the cable was crimped is unknown but is believed to have occurred during previous testing and maintenance activities. ~

The crimped signal cable was replaced and the monitor returned to service on October 28, 1986 following surveillance testing. This Special Report is being submitted pursuant to Technical Specification 3.3.7.5.

Event Date: 10/17/86 Event Time: 2045 CDEDlTlDEB RElDB TD DCCDBEEBCE Plant in OPERATIONAL CONDITION 1 with a Reactor power of 50% and a unit load of 500 MWe. Filtration, Recirculation, and Ventilation System (FRVS) Vent Radiation Monitor inoperable due to intermittent indication on the mid-range detector of the High Range Noble Gas Monitor.

DEBCBlETlDE DE DCCDEBEECE On October 14, 1986 during performance of an energy response determination test on the FRVS Vent Radiation Monitor, the mid-

-range noble gas detector exhibited. intermittent indication. At 2045 hours0.0237 days <br />0.568 hours <br />0.00338 weeks <br />7.781225e-4 months <br /> the monitor was declared inoperable and the I&C Department initiated troubleshooting and repair. Since the radiation monitor remained inoperable for more than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, the Radiation Protection Department .was notified to implement the preplanned alternate method of monitoring, if required. This monitor. is normally in the standby condition and is automatically placed in service during high radiation' conditions. During the period of inoperability these conditions did not exist, therefore, alternate monitoring was not required, hNhL1BlB DE DCCDEBEBCE Investigation of this event revealed that the mid-range noble gas detector intermittent indication was caused by a crimped cable which connects the detector output to the monitor electronics.

This crimp- resulted in a sporadic open circuit in the detector signal. The means by which the cable was crimped is unknown but is believed to have occurred during previous testing and maintenance activities. This Special Report is being submitted pursuant to Technical Specification 3.3.7.5.

Surveillance testing was completed and the monitor was declared operable on October 28, 1986. I&C and Radiation Protection Department procedures will be revised to include precautions relative to the proper installation of these cables following testing and maintenance activities. In addition, all technicians in both departments will be trained in the proper method of installation of the detector cables.
