APPENDIX A NOTICE OF VIOLATION Gulf States Utilities Docket: 50-458/86-27 River Bend Station License: NPF-47 During an NRC inspection conducted on August I through September 15, 1986, two violations of NRC requirements were identified.. The violations involved a failure to meet electrical specification requirements and failure to follow a surveillance test procedure. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the violations are listed below:

A. Failure to Meet Electrica1 Specification Requirements Criterion V, Appendix B of 10 CFR Part 50 requires that activities affecting quality be prescribed by and accomplished in accordance with documented instructions, procedures, and drawings. The licensee's approved' quality assurance (QA) program also reflects these requirements.

River Bend Electrical Specification 248.000 states that cable splices shall be n de in junction or pull boxes, condulet fittings in conduit with single circuits, or within equipment enclosures.

Contrarytotheabove,on-September splices for cables in a short section 4,(about4 feet)ofcablebetweenthe1986, it was found tha ,

conduit termination and the battery post to the Division III batteries were not enclosed as required by the instrr tions contained in electrical specification 248.000.

This is a Severity Level IV violation. (SupplementI.D.)(458/8627-01)

B. Failure to Follow a Surveillance Test Package Technical Specifications, paragraph 6.8.1.a. requires that written procedures shall be established, implemented, and maintained for activities covered by Regulatory Guide 1.33, Revision 2, February 1978.

'Section 8.b.2.q of Regulatory Guide 1.33, Revision 2 requires that surveillance test procedures be provided for " Emergency Power Systems."

River Bend Station Surveillance Test Procedure STP-203-1102, " Battery Weekly Surveillance Test For Battery IE22*S001 BAT," Revision 2 requires that the person (s) performing this procedure obtain and record the pilot cell numbers utilizing the last performed STP-203-1302. The cells with the lowest corrected specific gravities are to be used to assure battery operability.

1 Contrary. to the above, on September 12, 1986, it was found that the licensee had performed Procedure STP-203-1102 on September 3 and 8, 1986, without utilizing the pilot cells with the lowest corrected specific gravities as identified in the last performed Procedure STP-203-1302,

, which identified pilot cells 33 and 37 as having the lowest, corrected specific gravities of 1.210 and 1.208, respectively. However, the above listed performances of STP-203-1102 utilized pilot cells 37 and 52, and cell 52, had a corrected specific gravity of 1.212.

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violations if admitted, (2) the corrective steps which have been taken and the results achieved, (3)thecorrectivestepswhichwillbetakentoavoidfurtherviolations,and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
