This refers to the special safety inspection conducted by Ms. C. C. Casey of this office on February 24 through April 3, 1987, of activities authorized by NRC Special Nuclear Material License No. SNM-1426 and to the discussion of our findings with you and members of your staff on April 3, 1987.
The special inspection was conducted in response to your request to terminate the license dated January.16, 1987.
The enclosed copy of our inspection report identifies areas examined during the inspection. Within these areas, the inspection consisted of a selective examination of procedures and representative records, observations, facilities tour and interviews with personnel.
During this inspection, certain of your activities appeared to be in violation of NRC requirements as specified in the enclosed Appendix A.
A written response is required.
In addition to our concern for corrective action with respect to the specific violations noted, we are concerned about your management control system that has permitted these items to occur.
Consequently, in your reply to this letter please describe those actions taken or planned to improve the effectiveness of your management control over your licensed activities so as to preclude the recurrence of these or similar violations.
Enclosed is Appendix B, " Management Control" to provide you with some guidance for your response.
The responses directed by this letter and the accompanying Notice are not subjecttotheclearanceproceduresoftheOfficeofManagementandBudget as required by the Paperwork Reduction Act of 1980, PL 96-511.
