Requalification Program Evaluation Report No. 50-289/87-07 (0L)
From February 20 through March 12, 1987, the NRC audited the administration of
your annual requalification written, simulator, and oral examinations. At the
exit meeting held on March 12, 1987, the preliminary results of these evalua-
tions were discu; sed.
Areas examined during this evaluation are described in the NRC Region I Requal-
ification Program Evaluation Report which is enclosed with this letter. Within
the scope of this evaluation, the requalification training program was found to
be satisfactory.
However, two concerns were identified as a result of this
audit.
These are documented in Appendix A.
Your response by letter dated
April 1,1987 (H. HLkill to T. Murley) satisfies our second concern and your
commitment to document individual performance will be reviewed following the
next annual simulator requalification evaluations.
You are requested to re-
spond to the remaining concern by explaining what actions have been taken or
are planned to improve this area.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this
letter and the enclosure will be placed in the NRC Public Document Room.

APPENDIX A
Specific areas 'of concern regarding the implementation of the requaliffbation
program and.the applicable regulatory requirements are presented below. Refer-
ences in the parenthesis are paragraphs in the enclosed report.

No provision exists to ensure that the simulator evaluation which takes
place under the terms of your: requalification program requires the opera-
, tor to be examined in a position and at a level commensurate with the
broadest possible scope of responsibility for which that individual is
licensed. Additionally, one seniar operator oral examination observed by
the NRC was conducted at an R0 level of knowledge and this operator was
not evaluated as an SR0 darine the simulator examination. An SRO must be
evaluated at an SR0 level of knowledge and ability during all' phases of
the evaluation.
This requirement is implicit in 10 CFR 55, Appendix A,
4.a-e. (Section 5.). (0 pen Item 87-0732)

Evaluations made during the simulator portion of the requalification exam-
ination result in incomplete documentation of individual weakness neces-
sary to determine eligibility for operator license renewal. The require-
ment to keep individual records is contained in 10 CFR 55, Appendix A, 5.a
and their use to support an application for license renewal is contained
(Section 4). This concern has been satisfied-
by a licensee commitment to document individual performance during these
evaluations (0 pen Item 87-07-01).
