SAFETY SYSTEMS OUTAGE MODIFICATIONS INSPECTION
(INSTALLATION) 50-327/86-68 AND 50-328/86-68

This letter conveys the results and conclusions of a modification installation
-
inspection at Sequoyah Unit 2, which was conducted by the NRC as part of a
-
modified Safety Systems Outage Modifications Inspection (SS0MI). The inspec-
tion team was composed of NRC personnel and consultants, and was part of an NRC
program being implemented to examine the adequacy of the licensee's control of
modifications performed during plant outages.
The purpose of the installation part of the SSOMI program was to examine, on a
sampling basis, the installation of specific plant modifications accomplished
during the current outage at Sequoyah Unit 2.
This inspection concludes the
modified SS0MI program at Sequoyah Unit 2.
A special testing inspection,
which was conducted in March 1987, and which reviewed parts of the post-
modification testing program as well as other aspects of testing associated
with the outage, will be reported under separate cover.
Section 2 of the report is a detailed discussion of the installation inspec-
tion. The effort was hardware oriented and centered around specific modifica-
tions accomplished during the outage. Particular attention was directed toward
edequacy of installation procedures, conformance of modifications to require-
ments. traceability and control of material, and adequacy of safety-related
maintenance activities.
Section 1 of the report is a summary of the results of the inspection and the
conclusions reached by the team. The more significant concerns identified
were in the areas of (1) issue and installation of nonconforming or
untraceable materials and components, and (2) evaluations of ampacities and
bearing pressures for Flamemastic coated cable runs.
Our review gives indication of current generic problems in the area of material
control associated with all facets of modification work including procurement,
storage and installation of equipment.
In your evaluation of the enclosed
violation, you should examine the generic aspects of these deficiencies and
include in your response a discussion of root causes for these problems and
corrective actions to resolve the problems from this perspective. The resultsj-

of this inspection indicate that additional NRC inspections will be required
to be conducted in the areas of procurement and material controls prior to
restart of your Sequoyah units. These inspections will be scheduled pending
your implementation of corrective actions and resolution of concerns associated
with this inspection.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosures
will be placed in the NRC Public Document Room. The responses directed by
this letter and its enclosures are not subject to the clearance procedures of
the Office of Management and Budget as required by the Paperwork Reduction
Action of 1980, PL 96-511.
