The NRC welding team inspection was conducted April 20 through 24, 1987 at
the Browns Ferry Nuclear (BFN) facility. Members of the Office of Special
Projects.(OSP),Regionli, Office'NuclearReactorRegulation(NRR)andthree
consultants.made up the NRC welding team. This effort conshted primarily of
a detailed' review of the information contained in the TVA's Phase I report and
a review of selected design documents and drawings.

several inadequacies which TVA needs to address in the forthcoming Phase II
report. Enclosure 1 identifies the items which need to be addressed by TVA
and is an executive summary of the'results of this inspection and of
conclusions reached by the NRC. The enclosed inspection report (Enclosure 2)
details the scope, objectives and findings of the NRC inspection team and
identifies the areas examined during the inspection.

EXECUTIVE SUMMARY
A previously announced NRC welding team inspection was conducted at the
Tennessee Valley Authority (TVA) Browns Ferry Nuclear Power Station, Units 1,
2, and 3 from April 20 through 24, 1987. The objective of the inspection
was to review and assess-the adequacy and validity of the information reported
in TVA's Phase ~I report.
The NRC team noted no significant deviations from the results reported in
.
.TVA's Phase I report. 'The approach outlined in the report contained the
l
elements needed to determine whether TVA's licensing commitments have been
properly translated.into the governing specifications and drawings. However,
the NRC team identified several inadequacies that TVA must address in its

forthcoming Phase II report. These inadequacies include the following:
l(1) TVA had performed'very few reviews to determine how licensing commitments
were translated into the design documents before 1981. The sample
,
selection for TVA's Phase II inspection effort must include an adequate
'
number of pre-1981 fabricated welds to provide the basis required to make'
those determinations.
(2) The NRC. team found it hard to trace the _ origin of the allowable values
used for the structural component welding calculations. The engineering
calculations must show clearly the origin of the allowable stress values
used in the individual calculations.
(3)' The NRC' team identified instances in'which the drawings did'not provide a
traceable path to verify th t the design output documents accurately-
,
reflect the commitments T L made in the plants Final Safety Analysis
Report (FSAR) to meet the applicable construction codes. TVA's Phase II
report must. identify those items and provide adequate discussions about
whether or not the installed hardware meets the requirements of the
applicable construction codes.

(4) The NRC team identified several instances in which the FSAR commitments
were not reflected in the actual application. TVA's Phase II report must
identify those cases and indicate clearly whether the hardware is
,
!
deficient or whether the FSAR must be revised to reflect actual field
conditions.
(5) The NRC team found that the Phase I report included very few or no
,
examples for certain welding activities, such as welding on 1-inch
instrument pipe and welding on heating, ventilation, and air-conditioning
supports. The sample selection for TVA's Phase II inspection effort must
include an adequate number of 1-inch pipe and HVAC support welds to
provide the required basis to assess these two areas.
