RESPONSE
NRC INSPECTION REPORT NOS.
50-259/86-40, 50-260/86-40, AND 50-296/86-40 LETTER FROM G. G. LECH TO S. A. WHITE DATED MARCH 4, 1987 10 CFR 50, Appendix B, Criterion KVI, requires that measures to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material and equipment, and nonconformances are promptly identified and corrected.
Contrary to the above, a condition adverse to quality was identified and documented on Maintenance Request (MR) A-718016, dated October 15, 1986; however, the identified deficiencies were not corrected prior to closing out the MR on October 24, 1986, with the disposition of "all work / testing complete." The MR described that the flexible conduits installed from the junction boxes to the valve operators for the main steam isolation valves (MSIVs) had come apart and requested that the conduits be repaired or replaced and that the internals of the junction boxes and limit switches be inspected for corrosion. Valves 2-FCV-1-15, 27, and 38 were specifically identified.
An inspection on November 10, 1986 identified that conduit on valves 2-FCV-1-27 and 2-FCV-1-38 were still deficient.
This is a Severity Level IV Violation (Supplement I) and is applicable to unit 2 only.

Admission or Denial of the Alleged Violation (or Finding)
TVA admits the violation.

Reasons For the Violations (or Finding) if Admitted The root cause of this violation was a programmatic deficiency in dispositioning MRs.
In the case of this particular MR the individual working the MR was not able to identify the noted problem.
Because there was no mechanism in the maintenance program to require that the originator of an MR be consulted when the deficiency or concern cannot be located, the MR which was initiated to repair 2-FCV-1-15 and 27 and 38 was signed off even though several actions required by the HR work instructions were not completed.

Corrective Steps Which Have Been Taken and Results Achieved MRs 777217, 770709, 777203 have been written to correct the subject unit 2 MSIV conduit deficiencies.

Corrective Steps Which Will Be Taken to Avoid Further Violations (or Findings)
The procedure for initiating, reviewing, and dispositioning of MRs will be revised to require personnel working MRs to coordinate with the originator (or his representative) before dispositioning an MR when the deficiency or concern cannot be located.
