President 2600 Clifford Detroit, MI 48201 Gentlemen:

This is in reference to your application, dated May 13, 1986, to obtain a Nuclear Regulatory Commission (NRC) license for the pcssession and use of nickel-63 in a gas chromatograph.

In order to complete our review of your application, we need additional information on the following items:

1.

While under Mr. Caulton Ray, Jr.'s direction, Chemical Industrial Services, Inc. allowed its NRC license to expire, failed to respond to several notices of violation from the NRC, failed to respond to IP telephone notifications from the NRC regarding the expired license, and failed to perform procedures such as leak tests, which were requirements of its NRC license. Consequently, we will not authorize Mr. Ray to be the individual responsible for the radiation protection program on a license issued to ACIS Laboratories until such time as ACIS Laboratories can demonstrate that he can direct your program in compliance with NRC regulations and license requirements. Please resubmit the name(s) of the individual (s) who will be responsible for your radiation protection program.

In addition, specify the procedures you will have in place to ensure licensed activities will be performed safely and in accordance with regulatory requirements (e.g., delineation of duties and responsibilities, verification or auditing performance of safety related functions).

Chemical Industries Services, Inc. will need to respond to the NRC's April 29, 1984 and April 7,1986 Notice of Violation (enclosed) and indicate what corrective actions will be instituted to ensure that previous violations of NRC requirements (i.e., operating with an expired license and failure to perform leak tests) will be corrected and actions taken to prevent recurrence.

Material A.

Please identify the model numbers of the detector cells that will be utilized in the gas chromatographs (GC) identified in your application. The license issued to Chemical Industrial Services, Inc, lists only a detector cell for a Perkin-Elmer GC.

B.

Specify the number of GC's and c'etector cells you desire to possess.

4 Facility Please describe the location where your GC's will be stored or used. Item 9 of the attached guide will aid you in determining the type of location we consider acceptable.

Leak-testing Please indicate the type of leak-test you will be performing.

Item 10.3 of the attached guide specifies the information we need.
