Dear Dr. Grace:

By letter dated September 12, 1986, Duke responded to the violation which was identified in the above referenced report. In a subsequent telecon, Mr. Jape, et.

al. of your staff and Mr. McConnell, et. al. of my staff on September 29, 1986, it was determined additional information was needed for the NRC to fully evaluate Duke's response to the violation. The necessary information is contained herein.

The incident leading to the violation is an isolated event due to personnel error.

The refueling SRO should have called the control room and assured that the objects were logged, but did not due to the Westinghouse refueling advisor stating that he would make a note concerning the objects in his logbook. The Westinghouse advisor did not make a logbook entry and the SRO did not followup on the item.

The event is considered an isolated event due to the involvement of the Westinghouse advisor during the refueling process.

Corrective actions detailed in Duke's September 12, 1986 submittal will prevent recurrence of this type of event as demonstrated by the discovery of the Unit 1 fuel damage (reference LER 369/86-13 dated August 28, 1986) during the subsequent Unit i refueling outage.

Additionally, and independent of this incident, the Problem Investigation Report (PIR) system is to be instituted at McGuire. The PIR is a new system to ensure that all potential problems at McGuire (and Duke's other nuclear stations) are documented and corrected. The PIR form, which initiates the system, may be initiated by any plant personnel, thus minimizing the probability of a problem going unnoted. This system is presently scheduled to be in place by the end of this year at McGuire.
