A special safety inspection of the radiological controls program was conducted

on September 3 - 18, 1986 to determine the circunstances resulting in the

unplanned exposure to the hand of a worker on August 27, 1986. The areas
examined are identified in the enclosed inspection report and include methods

of handling sealed sources, labeling of radioactive material, instructions to
workers, and ALARA reviews.

The inspection findings indicate that there are inadequacies in your sealed

source custodial program. These inadequacies are discussed in Section 5.0 of

the attached report. You are requested to respond within 30' days of receipt
of this letter.
