NOTICE 0F VIOLATION AND PROPOSED' IMPOSITION OF A CIVIL PENALTY - SAN ON0FRE UNIT NO. 3 - RADIATION PROTECTION EA 87-63, DOCKET NO. 50-362 The NRR.. staff has reviewed the draft enforcement package concerning violations involving inadequate radiation protection precautions taken by-Southern California Edison Company involving the control of irradiated. fuel fragments during the period of. October 1986 through February 21, 1987.
Based on our review of the draft enforcement package, we conclude that the violations occurred, and were caused by a) A lack of procedures, training, surveys and recording of radiation measurements when faced with a substantial, potential hazard, and b) The less-than-timely reporting of a significant overexposure.
Accordingly, we concur with the proposed assessment of a Severity Level.III Civil Penalty for each violation. We also concur with your assessment that the licensee's good past performance in this area and the comprehensive corrective actions offset the multiple examples, prior notice and initial ineffective measures to cope with the difficulties encountered with the I
control of contamination involving irradiated fuel fragments, are appropriate mitigating factors.
