NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-327/87-24 AND 50-328/87-24)

.This refers to the Nuclear- Regulatory Commission (NRC) routine resident

inspection conducted by Kenneth M. Jenison on A)ril 6 - May 5,1987.

The

inspection included a review of activities autlorized for your Seguoyah

Lfacility.

At the ' conclusion of the inspection, the finding,s were discussed

with those members of your staff identified in the enclosed inspection report.

. Areas examined during the inspection are identified.in.the report.

These areas

included :' drawing . control, the operability look back program, .recent reactor

. coolant-. spill : events, and' the new employee concerns program.

Within these

areas, . the' inspection consisted of selective examinations of. procedures and

representative records

interviews with. personnel, and observation of

, activities in progress.,

The inspection findings 1 indicate that certain activities appeared to violate

NRC requirements.

Violation 50-327,328/87-24-03 involved a failure-to properly

-notify the NRC of the reactor coolant spill event of A)ril 29 1987.

Aside

r

from being a required licensee responsibility, keeping tie NRC informed on such

matters is vital to 'the' success of the Sequoyah restart effort.

Accordingly,

the violation

references to pertinent recuirements, and elements to be

' included in yo,ur. response are described in tie enclosed Notice of Violation.

Two Unresolved Items were identified in the area of drawing control.

These-

' items, which involve adequacy of primary and critical drawing lists, and'

"

adequacy of the temporary drawing change process, will require resolution

prior to restart.

An additional concern ' involving in consistency of drawing

. control programs at different TVA sites was also observed, We consider that

you should evaluate this concern from a corporate perspective and we will your

~ resolution of this. concern during subsequent inspections.

One Unresolved Item was identified in the area of arocedure compliance and

adequacy as '.it relates to control of operations.

ke are concerned with your

existing practices associated with procedure utilization system and equipment

status, hold orders, and testing as evidenced by recent events'and inspection

findings.

We will be addressing these areas with you at some future date.
