Gentlemen:
l This refers to the inspection conducted by Messrs. R. L. Cilimberg and W. Shier, and Ms. C. M. Abbate of this office on October 27-30, 1986, of your facility at Richland, Washington, and to the discussions of our findings with you and members of your staff at the conclusion of the inspection.
t The purpose of this inspection was to review the fabricatiion and inspection of fuel assemblies, computer code design activities and Quality Assurance (QA) program implementation at your Richland facility. Areas examined during the inspection included a review of fuel rod fabrication, fuel assembl fuel skeleton assembly, nondestructive testing, computer code (CC)y operations, verification, CC maintenance, CC error report handling, corrective action on previous inspec-1 tion findings and fuel problems reported at St. Lucie Unit 1, Yankee Rowe, and
- Big Rock Point. Areas examined during the . inspection and our findings are i discussed in the enclosed report. Within these areas, the inspection consisted of an examination of procedures and representative records, interviews with
, personnel, and observations by the inspectors.
During the inspection it was found that the implementation of your QA program
, failed to meet certain NRC requirements. Weaknesses were observed in the W nondestructive testing and the CC verification areas. Specifically, operators i used an unapproved procedure for radiographic testing of fuel rods and an unapproved coordinates chart for ultrasonic testing of tubing to be used for fuel cladding. Further, the RELAP5/M002 code implementation of the Moody critical flow model was determined to be incomplete in that the verification calculation did not include two phase flow conditions. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.
Please provide us within 30 days from the date of this letter a written state-ment containing: (1) a description of steps that have been or will be taken 4 to correct these items; (2) a description of steps that have been or will be taken to prevent recurrence; and (3) the dates your corrective actions and preventive measures were or will be completed. Consideration may be given to extending your response time for good cause shown.
