Millstone Nuclear Power Station, Unit No. 3
Responte to I&E Inspection Report No. 50-423/86-27 '
In Reference (1), Northeast Nuclear Energy Company (NNECO) was requested to
review three (3) items identified during an inspection conducted from August 18
through August 22,1986, by your office at Millstone Unit No. 3. A response was
to be submitted within thirty (30) days of receipt of Reference (1) on corrective
actions taken or planned in regard to the identified items.
NNECO hereby submits information concerning actions taken or planned in
regard to three items identified as 86-27-01, 86-27-04 and 86-27-06 in the
referenced inspection report.
1)
86-27-01 (Paragraph 4.3.1)
The equipment problems identified during the performance test must be
' ,
'
resolved to ensure PASS sampling capability. In addition, the schedule
for correcting these problems should be provided as requested during the
exit interview.
A.
Leakage in the dissolved gas sampling line.
Response:
1
A plant trouble report (4M3100340) has been generated. Any corrective
actions necessary will be completed by March 31,1987.
B.
Flushing pump tripped five times during performance test.

Response:
A plant modification request (3-86-506) has been generated.
An
engineering evaluation and recommendation for resolution is expected to
be completed by December 31,1986.
C.
Pressure indicator was slow to respond and did not stabilize.
Response:
The pressure indicator was serviced and it was found to function
properly.
D.
Operation of a valve (V-46) which isolates the containment
atmosphere sampling line was hampered for one hour when its
reach rod pin was sheared.
Response:
The pin was replaced.
A plant modification request (3-86-597) was
generated.
An engineering evaluation and recommendations for
resolution is expected to be completed by December 31,1986.
E.
PASS instrumentation was on a 2n year calibration frequency
which was in excess of the vendor recommended frequencies.
Response:
The calibration frequency has been changed to 18 months, which was
agreed to by the inspection team.
2)
86-27-04 (Paragraph 6.4)
o
Improve laboratory procedures for the handling and analysis of
high level samples.
o
Demonstrate that plant personnel can collect, replace and
transport samples within GDC-19 limits.
o
incorporate silver zeolite filters in lieu of charcoal prior to
switchover to silver loaded silica gel, in order to reduce potential
radiation levels due to retained noble gases and to assure a
representative sampling technique.
These items will be reviewed during a future inspection. In addition, the
schedule for this item should be provided as requested during the exit
interview.
Response:
An engineering evaluation is being conducted to address the above items.
It is NNECO's intention to complete the evaluation by December 31,
1986 and to implement the recommendation by March 31,1987.

86-27-06 (Paragraph 7.3)
The licensee was requested to include in their response to this inspection
a description of their intended actions to ensure station radiation
monitor calibration procedures were effectively
reviewed and
implem,ented. This item will remain open and be reviewed in a future
inspection.

Response:
The radiation monitor calibration procedures for Millstone Unit Nos.1,2
and 3 will be reviewed for consistency and correctness. It is NNECO's
intention to complete the review of these procedures by December 31,
,
1986 and to implement recommendations by March 31,1987.
