IDENTIFICATION OF OCCURRENCEz 23 Diesel Generator - Trip on High Jacket Water Temperature Event Date(s): 8/25/86 j Report Date: 9/23/86 This report was initiated ay Incident Report No.86-270 CQHPITIONS PRIOR TO OCCURRENCEr Mode 1 - Reactor Power 100% - Unit Load 1100 MWe DESCRIPTION OF OCCURRENCEr This special report describes a valid test failure involving 2B Diesel Generator occurring on August 25, 1986. This report is submitted for informational purposes in accordance with Technical Specification Surveillance Requirement 4.8.1.1.4 and contains the information required by Regulatory Guide 1.108, Revision 1, August 1977, Regulatory Position C.3.b.

Surveillance Requirement 4.8.1.1.4 states:

All diesel generator failures, valid or non-valid, shall be reported to the Commission pursuant to Specification 6.9.1.

On August 25, 1986, an operational test of 2B. Diesel Generator was in progress (in accordance with Surveillance Procedure SP(0) 4.8.1.1.2.,

" Electrical Power Systems - Emergency Diesels") to check for service water system leaks prior to tube bundle replacement of the Lube Oil Heat Exchanger and Jacket Water Heat Exchange:. Five minutes into the test, the diesel tripped on High Jacket Water Temperature.

APPARENT CAUSE OF OCCURRENCE:

2B Diesel Generator tripped as a result of the High Jacket Water Temperature alarm. The operators observed the "B" Diesel Cooling Service Water Throttle Valve (22SW42) sticking closed. The operators stroked the 22SW42 valve successfully and concluded the valve was operable. The diesel's operability was then tested successfully in accordance with surveillance procedure SP(0)4.8.1.1.2, "Slectrical Power Systems - Emergency Diesels".

The root cause of the diesel test failure was "B" Diesel Cooling Service Water Throttle Valve (22SW42) sticking which resulted in insufficient cooling of the diesel and the subsequent High Jacket Water Temperature alarm.

  • SPEdIAL REPORT 86-9 &MMJSIS OF , OCCURRENCE The High Jacket Water ~ Temperature trip is bypassed during SEC operation, hcwever, loss of jacket water cooling would have ultimately.resulted in failure of the Diesel Generator. Therefore,

=this diesel trip constitutes a valid test failure as described in Regulatory Guide 1.108. With one diesel inoperable, the remaining diesels are capable of providing power to the minimum safegoards equipment required for all accident and transient conditions.

CORRECTIVE ACTION 2 To determine the cause of valve 22SW42 sticking, the valve was stroked (successfully). The sticking phenomena could not be duplicated. No. 2B Diesel Ganerator was retested successfully in

, accordance with Operations Procedure SP(O)~ 4.8.1.1.2. As of the data 1

of this report, 2B Diesel Generator has been operated a number of times since the valid test failure on 8/25/86 without recurrence of valve 22SW42 sticking.
