This refers to the inspection conducted by E. H. Trottier of this office on September 8-12, 1986, of your facility at Oakland, California (and your supplier's facility, Woodward Governor at Fort Collins, Colorado) and to' the discussions of our findings with Bruce Guntrum and members of your staff at the conclusion of the inspection.
This inspection was performed as a result of a recently reported apparent deficiency in the material composition of two connecting rods in a diesel
. generator at San Onofre Nuclear Generating Station (SONGS).
In addition,-
this. inspection sought to followup on the actions taken by you and Woodward Governor Company as a result of the Grand Gulf engine overspeed event of November 1985. Areas examined during the inspection and our findings are discussed in the enclosed report. Within these areas, the inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspectors.
The inspectors determined that 'while the material composition of the subject connecting rods conformed to standards in effect when manufactured (ASTM A235E, 668D or AISI 1042), the rods were incorrectly stamped with a heat number traceable to a stronger steel (ASTM A237F, 668L or AISI 4142).
In reviewing the results of the TDI investigation into this matter, it appears that TDI has taken adequate corrective action by replacing the mismarked rods at San Onofre.
In addition, TDI established the location and verified the correct application of all similar connecting rods in their engines at other nuclear sites.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and the enclosed inspection report will be placed in the NRC's Public Document Room.
