Resident Inspection 50-213/86-27 (10/1/86 - 11/17/86)

This transmits the findings of the routine NRC inspection of the Hadda:n Neck Plant

on October 1 through November 17, 1986. These findings have been discussed with

Mr. R. Graves of your staff.

The inspection identified violations related to procedural compliance, calibration,

post maintenance testing, and containment integrity.

Your corrective actions for

the first three violations listed in the enclosed Appendix A were scheduled or

completed during this inspection and found to be satisfactory.

Therefore, no reply

to these three violations is required.

Thank you for informing us of the corrective actions documented in your letter to

NRC Region I dated October 14, 1986.

Those actions pertained to the findings of

Inspection 50-213/86-20, forwarded to you by our August 29, 1986 letter.

These

actions did not prevent a similar violation of containment integrity on October

8, 1986. This is Violation No. 4 of the enclosed Appendix A and is further dis-

cussed in Detail 4.3 of the enclosed inspection report.

Your October 24, 1986

letter to NRC Region I described this event and your current position regarding

manipulation of manual containment boundary valves. We agree that opening certain

manual containment isolation valves (CIVs) for surveillance may enhance safety.

That does not authorize deviation from the Technical Specifications (TSs) except

when approved in advance or in an emergency.

In non-emergency cases, your obli-

gation is to comply with the TSs and submit appropriate amendment and waiver re-

quests when needed.

Such requests have been granted in the past, and we will sup-

port justified ones in the future.

For Haddam Neck, TS 3.11 requires containment

integrity.

TS 1.8 requires manually operated CIVs to be closed for containment

integrity (except specifically exempted ones).

Opening manually operated CIVs

except as specifically authorized violates containment integrity and the associated

TSs, and will continue to be treated accordingly.

NRC Licensing has been consulted

and agrees with this position.

Please respond to the recurrent violation in ac-

,

cordance with Appendix A.

NRC review (Report Detail 5.d) of your actions responding to IE Bulletin 79-18,

Audibility Problems Encountered on Evacuation, found that you have not completed

paging system improvements to assure personnel in all plant areas can be notified

of plant emergency conditions.

In your reply to this letter, please describe the

paging system modifications to be made and provide your commitment date for their

completion.
