This inspection was conducted in response to a 10 CFR Part 21 report received from Sacramento Municiral Utility District (SMUD) concerning Wilmar Electronics supply of underpower relays to the Rancho Seco Nuclear Power Plant.

Ouality Control Wilmar Electronics manufactures a wide range of catalog and custom order l
electronic relays and sensors. A substantial number of these relays have been supplied to nuclear power plants or their vendors and in most cases l
were supplied as commercial grade equipment. Of the purchase orders reviewed, none invoked 10 CFR Part 21, however, a number of purchase orders stated the relays were to be able to meet various seismic requirements. Wilmar has not provided any certification of the relay's ability to withstand a seismic event, but similar Wilmar relays have been tested by Wyle Labs as part of seismically qualified control panels.
Two drawings reviewed, Wilmar drawing 21-441, "Close Differential Relay Three Phase Adjustable Model 0101-17X" and drawing 71-442, " Frequency Relay Model 20-050-22X" called out 10 CFR 50 Appendix B quality assurance criteria. These relays were supplied to SMUD in dure of 1980 Wilmar's Ouality Control Manual dated June 3,1985, was reviewed and no deficiencies were noted. Wilmar relies on a testing program in which 100% of relays are functionally checked to ensure a quality product.
Each relay is inspected by an inspector who applies his individual stamp to a satisfactorily tested component. Upon completion of the functional test, a dielectric test is performed to check fur proper insulation integri ty. Wilmar's test eauipment control was reviewed and found to bo 1

PAGE 3 of 4 satisfactory. All equipment calibritions on the master calibration list were found to be up to date with a selective check performed on individual pieces of test equipment confirming this observation.
Design changes were found to be handled properly with changes documented and signed off on the appropriated drawings. Selective production depart-ment folders were. reviewed and were found to contain the required up-to-date Schematic Diagrams and Bills of Material.
_Underpower pelays Supplied to SMUD On July 30, 1986, the NRC received a 10 CFR Part 21 report from SMUD stating that underpower relays Model 21-71 supplied by Wilmar were defective.
The letter stated several of the 16 relays that were ordered contained overheated components, damaged circuit boards, bad solder connections, etc. Documentation pertaining to the supply and repair of these relays was reviewed by the NRC inspector.
The relays in question were originally purchased by Wolfe and Mann who installed them in panels which were seismically qualified by Wyle 1. abs in test report 42401-1 dated December 18, 1972.
The purchase order for the supplied relays required them to be Seismic Class 1.
Af ter receiving the panels from Wolfe and Mann, SMUD sent the underpower relays back to Wilmar a number of times. The first time was for a change in the relay's control voltage source from en internal to an external source. This change was performod on all relays.
Subsequently, the relays were sent back for a number of in some instances, bad zener diodes were found while in
- reasons, other instances the relays were tested and found to be in proper working order.
Wilmar noted that SMUD had attempted repairs on the relays as noted by the observed installation of new, dissimilar components.
in 1984, af ter failure at SMUD Wilmar made a design change to the relay by replacing one apparen,tly undersized resistor with two resistors whose combined power dissipation was considerably greater.
SMUD did not request this change be performed on all 16 relays. Wilmar stated the design change along with the SMUD repairs may have been the reason SMUD has reported the relays no longer conform to the original Wilmar supplied drawings.
Wilmar has indicated the underpower relays were not designed for a 40 year life and that they should now be replaced with relays of a newer design. Wilmar has similarly recommended replacing the Model 21-172 phase balance monitor relays which were suppifed to SMUD in 1972.
