Dear Sir:
May 20, 1986 At this time we wish to amend our NRC Materials License, License Number SNM-1435.
The licensed source will no longer be under the supervision of David Solomon.
As per the conversation with our Austin Colburn on May 12, 1986, we have arranged for transfer and storage of our source to return to Ohmart Corporation.
It is our intention to discontinue all activity involving this source and store the material at the Ohmart Facility until such time as we may arrange for disposal of same.
Copies of Documents enclosed include our License, Ohmart License sent to us, Ohmart Quote and Shipping Manifest by Ohmart.
If there are any further documents required, please notify me.

The test shall be capable of detecting the presence of 0.005 microcurie of j
radioactive material on the test sample.
The test sample shall be taken from i
j.i the sealed source or from the surfaces of the device in which the sealed source i
e is permanently mounted or stored on which one might expect contamination to i
accumulate.
Records of leak test results shall be kept in units of microcuries and maintained for inspection by the Conmission.

If the test reveals the presence of 0.005 microcurie or more of removable l
contamination, the licensee shall ~immediately withdraw the sealed source from j
l use and shall cause it to be decontaminated and repaired or to be disposed of in s
j accordance with Commission regulations.
A report shall be filed within 5 days i.
