Dear Mr. Keppler:

Pursuant to the request of Mr. N. Gautam of your staff on August 1, 1986, this letter transmits the results of the actions we have taken regarding IE Information Notice 86-03, " Potential Deficiencies in Enviren= ental Qualification of Limitorque Motor Valve Operator Wiring."

This information is available in File 8I of our Central Equipment Environmental Qualification File, both at the plant and at our corporate offices in Columbus, Ohio.

In response to this Information Notice, we initiated a formal walkdown program to inspect the internal control wiring in the environmentally qualified Limitorque motor operators at Cook. The attachment to this letter is a list of all of the Limitorque operators which have been inspected at the Cook Plant. The details of our inspection are on file at the plant.

Of the 146 Limitorque operators inspected, seven were found to contain jumper wires made of undocumented material. As we stated in our letter AEP:NRC:0775AF, the undocumented wire in those seven operators was immediately replaced with wire which had been qualified in accordance with the requirements of 10 CFR 50.49.

The following table provides the location and function for each of the seven valves found to contain undocumented wire.

During that period of time, we had a special audit on environmental qualification by the NRC.

The persons participating in that audit provided further suggestions as to the requirements of the Limitorque valves. As a result of their suggestion, we took additional action as summarized in our letter AEP:NRC:0775AE.

In parallel with this effort, we also performed safety evaluations of the impact of what we had found. On four of the valves (2-WM0-714, -724,

-726, and 1-IMO-910) we foun'd no impact whatsoever, since they were not located in an area that would be affected by a HELB and therefore did not require special environmental qualification for high-energy line breaks. On the three valves inside containment, we found there was no impact on IMO-54 because the valve was not functionally required to change position following a high-energy line break inside containment.

On two valves, IMO-315 and IMO-316, we found a safety impact, since these val es are required for the change from cold-leg to hot-leg recirculation 24 haurs following a postulated loss-of-coolant accident.

Since the affected valves were restricted to one of the safety trains, :lue other, redundant train would have been available for use in a design-basis accident.

However, since the problem was restricted to lack of documentation with the jumper wires, we conducted an engineering evaluation to determine whether the valves could

Mr. Hzrold R. D5nton AEP:NRC:0775AH have performed their intended safety function. Our conclusion was that they could have performed this function of switchover from cold to hot leg recirculation. On the basis of our evaluation, we believe that the event did not constitute a significant safety hazard.

This document has been prepared following Corporate procedures which incorporate a reasonable set of controls to insure its accuracy and completeness prior to signature by the undersigned.
