NOTICE OF VIOLATION Illinois Power Company License No. NPF-62 As a result of the inspection conducted on April 6 through May 18, 1987, and in accordance with the " General Policy and Procedure for NRC Enforcement Actions",

10CFR Part 2. Appendix C (1985), the following violations were identified:

1. Technical Specification 4.0.4 requires that entry into an OPERATIONAL CONDITION or other specified applicable condition shall not be made unless the Surveillance Requirement (s) associated with the Limiting Condition for Operation have been perfonned within the applicable surveillance interval or as otherwise specified.

Contrary to theperforming 1987, without above, Operational Condition the surveillance 2 was entered requirements (channelon February)26, checks associated Wit) the following Limiting Conditions for Operation:

- LCO 3.3.2 for Containment and Reactor Vessel Isolation Control system isolations

- LC0 3.3.5 for Reactor Core Isolation Cooling system isolation

- LCO 3.3.2 for vessel high pressure permissive for RiiR shutdown cooling

- LC0 3.3.2 for Reactor Water Cicanup system isolation on high area temperatures.

This is a Severity Level IV violation (Supplement I) (461/87015-02(ORP)).

2. Technical Specification 6.8.1.d requires that written procedures shall be established for surveillance of safety related equipment.

Contrary to the above, written procedures were not established to perform required surveillance channel checks on the following safety related equipment

- Containment Pressure High instruments VG-145 and VG-147

- Reactor Vessel Water Level LOW-LOW Level 2 instruments 1021-N692A, D, E,& F

- Reactor Vessel Pressure High (RHR cut-in permissive) instruments 1021-N679A, D, C,& D

- Equipment Area Temperature instruments E31-N626A & D and -N610A & D.

- Reactor Vessel Water Level 8 instruments IC34N004A, 0, C,& D.

This is a Severity '.evel IV violation (Supplement 1)(461/87015-03(DRP)).

The inspection showed that action had been taken to correct the identified violations and to prevent recurrence. Consequently, no reply to the viola-tions is required and we have no further questions regarding this matter.
