- HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING NOVEMBER 21, 1986 Perry Nuclear Power Station. Unit 1 On November 13, 1986, the United States Court of Appeals for the Sixth Circuit issued an order requiring that the licensee not operate the Perry Plant at power levels in excess of 5% of full power, pending a hearing on the merits

. of the State of Ohio Petition. The Court has directed an expedited briefing schedule for this case. Ohio and intervenor filings are due on November 19, 1986, with the NRC and licensee filings due on November 24, 1986.

following authorization by the Commission on November 7, 1986.

Description:

The contractor shall conduct cost analysis studies to identify, derive and analyze the economic impact of proposed changes in regulatory requirements and the costs to licensees for replacement power when a plant is shutdown.

Description:

The objective of this procurement is to assess NRC's Financial Assurance Program for NMSS licensees to determine the adequacy of the existing regulatory framework to assure that adequate financial assurance and sufficient coverage are in place and maintained at all times.

NOVEMBER 21, 1986 ENCLOSURE A

0FFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending November 21, 1986 Crystal River Unit 3 Shutdown Crystal River Unit 3 was shutdown on November 12, 1986 due to a failed CROM motor. During this outage, the licensee intends also to correct the main feed pump problem and to replace a weeping pressurizer safety valve and find and repair a leak in the waste gas system in the containment.

As a result of the waste gas system leak and the plant's inability to purge during operation (the large valves are closed in Modes 1-4), the radiation level in containment has risen to 1.4 R/hr.

This had caused the licensee to consider requesting on-time authorization to purge containment during Modes 3 and 4 to assist in leak location and minimize down time. The licensee subsequently determined this request would not be pursued, but might consider such a request in the future.

According to preliminary reports, main feedwater difficulties at low power levels during the shutdown process caused actuation of the emergency feed system. The EFW system performed as designed, except that the steam driven pump tripped on overspeed. There has been at least one other recent instance of control difficulties in the main feed system at low flow conditions causing EFW actuation.

Hope Creek Generating Station On November 10, 1986, Hope Creek attained, and was operated at 100% pcwer for the first time. After attaining 100% power, power was reduced in order to conduct additional recirculation pump speed fine tuning. Hope Creek is in the final stages of the Power Ascension Test Program. As part of the power ascension program, the licensee will conduct a full MSIV closure on November 16, 1986, and a load rejection from 100% power on November 19, 1986. The 100 l hour warranty run is scheduled to be completed by December 13, 1986. Follow-l ing successful completion of the warranty run, the unit will be declared i commercial.

River Bend Station On October 24, 1986, Gulf States Utilities (GSU) informed the Secu'rities 'and Exchange Commission (in a Form 8-K filing): "Without rate relief or funds from external financing, the Company (GSU) will be unable to meet its obliga-tions and may have to seek protection from its creditors under the Bankruptcy Code."

On June 29, 1986, the licensee informed the staff of its potentially adverse financial situation but stated that GSU was fulfilling their financial obliga-tions related to River Bend operation.

NOVEMBER 21, 1986 ENCLOSURE B

The Office of State Programs is monitoring the financial status of the River Bend Station Licensee.

Perry Nuclear Power Station, Unit 1 The full power operating license for Perry Unit I was issued to Cleveland Electric Illuminating Company, and others on November 13, 1986, following the Commission's vote on November 7, 1986 authorizing such issuance. The plant had been issued a low power license on March 18, 1986 and completed nuclear heatup on October 22, 1986. Since that date, the unit has been in a maintenance outage which is expected to be completed on November 16, 1986.

Shearon Harris i Carolina Power & Light (CP&L) began loading fuel into the Shearon Harris Nuclear Power Plant, Unit 1, reactor vessel at about 5:00 p.m. on November 17, 1986. As of 8:30 a.m. November 18, 1986 22 of the 157 total fuel assem-blies were loaded. Fuel loading and completion of cold shutdown testing is expected to be completed by early December. CP&L anticipates the completion of low power testing and be ready for full power operation by early January 1987.

Millstone Unit 2 Emergency Diesel Generator Relay, Seismic Qualifications On November 14, 1986, the licensee declared both Millstone Unit 2 EDG's inoperable due to a lack of seismic qualification of the differential current relays on the EDGs' output breakers. Millstone Unit 2 is currently in a refueling outage that began in September 1986 and will end in December 1986.

The licensee expects to receive new seismically qualified relays from General Electric and have these relays installed prior to restart.

c Pilgrim Nuclear Power Station On November 19, 1986, at about 8:00 a.m., with the unit in cold shutdown for  :

refueling and maintenance, the plant lost offsite power. At that time, the '

Boston area was covered with heavy snow and winds gusting to 60 MPH. Both the emergency diesel generators loaded on system components in sequence as required. The shutdown cooling subsystem was inoperable for a little over 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />. The power from the two 340KV offsite sources to the switch yards was restored at 11:03 a.m. and 11:26 a.m., respectively. At that time, non-vital loads were switched to the offsite power but the vital loads were kept on the  :

two diesels. At 12:38 p.m., the two diesels were taken off the line and the offsite power was completely restored to the station. The licensee has not determined the exact cause for this event.

Arkansas Nuclear One, Unit 1 During the current refueling outage for ANO-1, Arkansas Power and Light Company disassembled and inspected "A" Reactor Coolant Pump in response to their commitment to perform inspections of the RCP shafts and impeller cap screws and torque pins. With the rotating assembly of the pump removed from the pump casing, the licensee performed the 10-year inservice inspection of the pump casing. Radiography of one vertical weld in the pump casing showed a weld defect about six inches long. On comparing the radiograph with a

, computer enhancement of the original radiograph of this weld, the licensee i determined that the weld defect has been in the pump casing since the initial operation. Further, the weld defect h4s not changed. The licensee has also reviewed a computer enhancement of the' original radiograph of the welds of RCP "B" and discovered a defect. The such ar. described. TheCodewouldass$3MECodedoesnotaddressasituation ume that a preservice defect would be removed. The Code specifies actions required for inservice discovered defects which include analyses of the weld defects and augmented inspection plans for the affected pumps and the other pumps. The licensee has submitted fracture mechanics analyses of the "A" and "B" pump weld defects, but has not identi-fied the defects as inservice defects and thus does not propose additional action. The staff met with the licensee at the ANO site on November 20, 1986, to review radiographs and other techniques for examining the welds and to discuss what is further needed by the staff to resolve this issue before startup of ANO-1, which is currently planned for early December 1986.

Lim (rick l The ASLB issued a Supplement to its Fourth Partial Initial Decision on November 10, 1986. The Supplenent addressed an issue remanded to the ASLB by thr/ Appeal Board on ,the adequacy of the telephone call-up system at the State Ccbrectional Institee at Graterford to notify security personnel so that an evacuation of the SCIG can be accomplished if required by an emergency at the Limerick Plant.

The licensing board has found that arrangements in place for notification and mobilization of off-duty correctional officers meet the Commission's regulatory requirements and provide adequate assurance that adequate protective measures for the Graterford inmates can and will be taken.

NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT FOR NOVEMBER 17 - 23, 1986

1. DEFUELING

- Topographic mapping and video examination of the reactor core debris bed were done November 22, 1986. These are the first examinations after the bulk core drilling.

- Samples of the drilled debris bed were placed in canisters for early shipment to the Idaho Na'ional Engineering Laboratory. Some chunks of material have weighed 200 - 300 pounds.

- The first attempt at removing broken drill bits and drill string were unsuccessful. These pieces will be handled as they are encountered.

- Fuel debris loading will begin this week.

2. ENVIRONMENTAL MONITORING

- US Environmental Protection Agency (EPA) sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and the City of Lancaster Agreement.

- TMI water effluents are sampled from the station (Units 1 and 2) discharge and analyzed by EPA. Gamma spectrum analyses of the seven daily composited samples (November 1 - 8, 1986) indicated no TMI-2 related radioactivity.

- EPA's gamma spectrum analysis of the NRC's TMI outdoor air sample for November 13 - 20, 1986 showed no reactor related radioactivity.

- The Lancaster water works intake composited seven daily samples (November 2 - 8, 1986) ganma spectrum analyzed by EPA showed no reactor related radioactivity.

3. AUXILIARY AND FUEL HANDLING BUILDING ACTIVITIES

- Reclamation of Defueling Water Cleanup System (DWCS) filters continues.

- Fuel handling building ventilation system modifications are in progress.

- Pre-operational tests of the DWCS and Sediment Transfer systems are in progress.

- Fuel canister preparation for shipment is in progress.

- Preparations and initial data collection ace in progress for a gross flush of the reactor building basement.

- Installation of an ultraviolet light / hydrogen peroxide device for water treatment has begun.

Access Authorization Policy Statement and Industry Guidelines The Access Authorization Policy Statement and Industry Guidelines were forwarded to CRGR on November 12, 1986. CRGR has scheduled review for December 4, 1986.

Closure of Sheffield Negotiations between the State of Illinois, US Ecology, Sheffield facility site operator, and the NRC have been delayed by approximately one month. (The ASLB has been informed of this delay.) Currently, the staff is reviewing a copy of the State's proposed settlement agreement mailed to NRC counsel on November 14, 1986. It is unknown at this time if US Ecology agrees with the State's proposal.

For unrelated reasons, the granting of Agreement State status has also been delayed by one month, until about February 1, 1987.

Meeting with EPA The Director, NMSS will meet with Winston Porter (EPA) on November 21, 1986 to review the progress of ongoing NRC/ EPA interactions on Mixed LLW issues and discuss the following specific items:

NOVEMBER 21, 1986 ENCLOSURE C

. 2 Policy Meeting on Mixed Low-Level Waste with State of Washington NRC staff sponsored a policy meeting in Seattle, Washington on November 13, 1986 to examine technical, procedural, and administrative problems that may arise due to dual regulation of Mixed LLW at the Hanford Disposal Facility, Richland, Washington. Representatives from NRC, EPA, EPA Region X, and the State of Washington participated in the discussions. Representatives from Nevada and South Carolina, which host the other two LLW disposal facilities, attended the meeting as observers. The State of Washington is committed to the following goals:

Protect public health and safety and the environment in the State of Washington.

Meet the LLRWPAA implementation schedule.

Develop an approach capable of accomodating state-of-the-art technology for management and disposal of waste.

The State of Washington is committed to make the dual regulation work, and is progressing satisfactorily to resolve regulatory dif ferences as applicable to Mixed LLW.

Office of Inspection and Enforcement Items of Interest Week Ending November 21, 1986

1. Items of Interest
a. Pilgrim The Pilgrim site t xperienced a loss of offsite power for approxi-mately 3 1/2 hours on 11/19/86. All three offsite power lines were lost at the same time. The cause is still under investiga-tion, but problems existed in both the switchyard and the grid.

l 0FFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending November 21, 1986 2D/3D Thermal fluid mixing experiments ranging from 1/5 - scale to 1/2 - scale have been performed at Purdue, CREARE Incorporated, Imatran Voima Oy of Finland and HDRofGermany(fulllength). These experiments have shown that the cold emergency core coolant (ECC) injected into the cold leg mixes well with the hot primary coolant so that the effects of thermal stratification are greatly minimized in the downcomer region. This mixing phenomena significantly reduces the likelihood of pressurized thermal shock (PTS). To confirm this result at a full scale test facility, a fluid mixing test was performed at the German Upper Plenum Test Facility (UPTF) under the 2D/3D Program. The UPTF test results were indeed consistent with the small scale test results. These favorable experimental results provide further technical confirmation to help close the PTS issue. The UPTF fluid mixing test results were reported at the 14th Water Reactor Safety Research Information Meeting on October 30, 1986.

ROSA IV NRC has a cooperative agreement with the Japan Atomic Energy Research Institute (JAERI) to obtain data from the ROSA-IV test facilities. After the TMI accident, JAERI designed and built the Two-phase Test Facility (TPTF), and Large Scale Test Facility (LSTF). Both the TPTF and LSTF are components of the ROSA-IV program. The latter is the largest thermal hydraulic test loop (i.e.,

1/48 scale) in the world for investigating small break loss-of-coolant accidents (SBLOCAs) and cperational transients. The LSTF is a full height, I integral test facility; all the major components of the primary and secondary l systets of a PWR with a U-tube steam generator are modeled. The LSTF can run l various break sizes up to 10 percent and the break may be located at several different locations in the system. The TPTF is a separate effects test

' facility for the study of two-phase flow and heat transfer in rod bundles, and flow regimes in horizontal pipes. Both the LSTF and TPTF can operate at the normal operating conditions of a PWR.

The USNRC participation in the ROSA-IV program allows: (1) for the input of test types and test conditions for problems of PWR analysis for licensing, and  !

(2) for access to the test data which can be used for verification of NRC safety analysis codes. A ROSA-IV coordination meeting was held with JAERI staff on October 31 and November 3, 1986.

Upon the NRC's request, five 5 percent SBLOCA experiments were conducted in FY86 in the LSTF to investigate the core level depression and liquid holdup phenomena. The phenomena were first observed in the Semiscale Test Facility and had the potential to become a regulatory issue. The five LSTF experiments confirmed that the core liquid level depression and liquid holdup phenomena were not peculiar to the small-scaled Semiscale facility. However, the LSTF test results showed that the core heatup was not very high, and the heater rod temperature irrediately turned around when the break was uncovered, allowing the steam discharge from the primary system. To resolve this liquid holdup issue, further analysis of the phenomena in PWRs with NRC's best estimate codes will be conducted.

Another issue to be addressed in the ROSA-IV program is to determine whether the ECCS can keep the core covered after the accidental rupture of several instrument lines in operating pressurized water reactors. During FY86, the NRC analyzed the consequences of instrument tube ruptures using the RELAP5 code.

The results showed that the plant emergency core cooling system will prevent core uncovery if no more than 11 instrument tubes are ruptured. In order to assess RELAP5 calculational uncertainties, the NRC recorrended instrument rupture tests to be conducted in the ROSA-IV LSTF facility in FY87. Results of the tests will be used to assess the ability of the TRAC and RELAPS codes to predict plant behavior for vessel instrument line breaks.

Multi-Loop Integral System Test (MIST) Program The MIST facility is an integral test facility designed to simulate thermal l

, hydraulic aspects of transients in reactors of the Babcock and Wilcox design.

! The data will be used in assessing NRC thermal hydraulic codes. A MIST technical meeting was held at the B&W Office in Bethesda, Maryland on November 6 and 7, 1986. The purpose of the meeting was to discuss the criteria to be used for selecting tests for post-test analyses using the RELAP5, TRAC-PF1 and RETRAN codes. A set of criteria for selecting tests for post-test predictions was developed at the end of the meeting. B&W will produce a matrix that will catalogue tests against the set of criteria. Based on the matrix, B&W will recomend to the Program-Management Group by mid-December,1986, the proposed post-test analyses for the RELAP5 code.

3 Currently, the facility has completed the installation of the continuous vent line between the upper head of the reactor vessel and one of the steam generators. The continuous vent line is needed for MIST to conduct tests for the Toledo Edison Company. The cost associated with the continuous vent line tests was paid by the Toledo Edison Company. The last group of tests on Reactor Coolant Pump operation is scheduled to begin in mid-December, 1986.

Plant Risk Status Information Manacement (N-PRISIM) System A Plant Risk Status Information Management (N-PRISIM) system has recently been installed in room P-215 of the Phillips Building for generic use. This particular PRISIM version is a personal computer program that presents PRA results and related information for the ANO-1 Unit-1 nuclear plant in a clear and conc.'se format for use by licensing and inspection engineers. A major capability of PRISIM involves the calculation of the change in core damage frequency as plant systems, component failure rates, and accident initiating event frequencies are changed, based on user inputs. Installation of the prototype ANO-1 PRISIM at NRR is intended to create a feedback mechanism from NRR to RES which will allow PRISIM capabilities to be optimally tailored to NRC needs as additional systems are built in the near future, e.g. for the Peach Bottom plant.

J. F. Costello's Trip to Europe Dr. J. F. Costello, RES, visited offices of the Nuclear 1 Installations / Inspectorate, the Central Electricity Generating Board, and the i United Kingdom Atomic Energy Authority in England. These agencies are participating in our research program on containment performance under severe accident conditions. The discussions centered on progress of the work these organizations have undertaken for the upcoming test to failure of a 1/6 model l

of a reinforced containment and initial thoughts about how to project the insights that will be gained from the model test to the behavior of prestressed concrete containments.

l R. Kenneally's Trip to Europe Mr. Roger M. Kenneally, RES, just completed a trip to Yugoslavia, Germany and France to explore the extent of foreign participation in the NRC Seismic Research Program. In Skopje, Yugoslavia, Mr. Kenneally met with Professors Jurukovski and Petrovski of the Institute of Earthquake Engineering and Engineering Seismology. The staff at the Institute has performed tests on electrical equipment that may complement those performed on the NRC's Equipment Qualification and Component Fragilities Programs. Also, strong ground motion data from earthquakes that have and may occur at the Krsko Nuclear Plant would add to our knowledge of strong ground motion propagation and the resultant response of buildings and systems. The Yugoslavians are interested in cooperating; a more detailed description of the research program will be l prepared by both countries including areas to be explored for cooperation.

In Darmstadt, Germany, discussions with Professors Bouwkamp, Konig, Otes and Worner at the Technische Hochschule Darmstadt centered on a possible exchange of data on the testing of reinforced concrete. The data from the German research may provide insights into the large experimental-analytical differences currently observed in the NRC's shear wall test program.

Discussion with M. Hoffmann and MME Ganterbein of the Commissariat A L'Energie Atomique in France also focused on analysis and testing of reinforced concrete.

A possible arrangement discussed was that in en exchange for test data, the French would perform analytical studies to see if their analytical methods would also show the same analytical-experimental difference that the NRC contractor is reporting.

In all cases, the meetings represerted the first step towards a formal exchange. In the case of the French and Germans, the exchange could be done

~under current agreements.

5 RES RULEMAKING ACTIVITIES Definition of High-Level Radioactive l!aste (HLW) in 10 CFR Part 60 This advance notice of proposed rulemaking (ANPRM) seeks to revise the definition of HLW in Part 60 to reflect certain changes in the legal definition of HLW contained in the Nuclear Waste Policy Act of 1982. A revision of the definition of HLW would reduce uncertainty as to final disposal of certain above Class C wastes, DOE's plans for a geologic repository, State plans for regional compacts to manage low-level waste, and costs of waste disposal for certain waste generators. The possible need for development of new technologies and facilities to dispose of certain types of wastes could be affected.

The Commission paper, SECY-86-328, submitting the advance notice of proposed rulemaking for approval to publish in the Federal Register, was forwarded to the Commission on November 7, 1986.

1. AE00 Technical Review Report, AE0D/T608, " Hydrogen Fire and Failure of Detection System" was issued on November 17, 1986. The study documents the review of an event at a foreign reactor which involved a leak in the unit's electric generator hydrogen system followed by an explosion and a fire. Due to a design deficiency in the fire detection system that allowed too many detectors to be installed in a detection loop, the fire detection system did not respond correctly during the event - instead of annunciating a fire, it . indicated an "out of order" condition. The fire was detected promptly inspite of the detector system problems, and the plant was shut down safely. The study was initiated to assess whether such a fire detection system design could exist at U.S. nuclear plants since such a design deficiency potentially would be significant because the detection system would not perform its design function during a fire.

Based on a review of recent operational events in domestic plants involv-ing the fire protection system, such a design deficiency was not identi-fied in U.S. nuclear plants. Although a review of detailed design draw-ings of fire detection systems installed in specific U.S. plants was not performed, the report concludes that a large fire, that renders the fire detection system incapable of properly functioning, remaining undetected is a low possibility.

The staff should also consult with the Commission prior to going ahead with a generic letter.

The Commission requested that the staff provide estimated core melt frequencies for those plants identified in the briefing slides. The staff was also requested to provide its rationale for not pursuing rulemaking as a preferred solution to address the staff's concerns.
