NRC INSPECTION REPORT NOS. 50-327/86-53 AND 50-328/86-53

This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by

G. A. Belisle on September 29 - October 3, 1986.

The inspection included a

review of activities authorized for your Sequoyah facility. At the conclusion of

the inspection, the findings were discussed with those members of your staff

, identified in the enclosed . inspection report,

~j. Areas examined during the inspection are identified in the report. Within these

areas, the inspection consisted of selective examinations of~ procedures and

rep,resentative records, interviews with personnel, and observation of activities

in progress.

The violation described in the enclosed Inspection Report is under consideration

for escalated enforcement cction. Accordingly, no Notice of Violation is being

issued at this time, and no response to this letter is required. However, please

be advised that the number and characterization of violations described in the

enclosed Inspection Report may change as a result of further NRC review.

You

will be advised by separate correspondence of the results of our deliberations on

this matter.

The problem identified in this inspection report is viewed by the NRC as a

failure to successfully implement your established corrective action system.

This problem was discussed during a meeting between Region II and TVA management

on October 29, 1986. During this meeting TVA stated that proposed corrective

action for this issue was being pursued end presented an outline for this

,

corrective action. TVA is requested to document this outlined corrective

action, including specific dates for completion, and forward this information

to the NRC. Prior to the restart of either Sequoyah unit, the NRC intends to

verify that your actions have been successful.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this letter and its enclosure

will be placed in the NRC Public Document Room.
