s DUKE POWER COMPANY McGUIRE NUCLEAR STATION VIOLATION RESPONSE Violation 50-369/87-12-01 and 50-370/87-12-01 Technica1' Specification 6.8.1. requires that. written approved procedures be established,' implemented'and maintained covering the operation and maintenance of safety related plant equipment.

Operating Procedure OP/1/A/6250/02 " Auxiliary Feedwater System", enclosure 4.5 specifies the position of valves 1 CA-153 and 1 CA-154 among others, to place the system in standby readiness.

Operating Procedure OP/0/A/6100/09 " Removal and Restoration of Station Equipment" controls the removal of station equipment from service when the removal of that equipment is not covered by an established operating procedure. The procedure also provides the operators.with a status of all out of normal operating conditions by maintaining in the Control Room the procedure checklist until the equipment is returned to service.

Contrary to the above, on April 8,1987, the inspector identified that valves 1 CA-153 and 1 CA-154 were not in the positions specified by procedure OP/1/A/6250/02, nor was there an R&R controlling their misalignment.

This is a Severity Level V (Supplement I) Violation.

RESPONSE

1.

Admission or denial of violation:

The violation is admitted as stated.

2.

Reason for the violation if admitted:

This auxiliary feedwater storage tank and associated valves of the CA '

system can be operated properly with the tank aligned to either Unit 1 or Unit 2.

Operators afstakenly thought the procedure (OP/1/A/6250/02) allowed alignment to Unit 1.

Even though the system was not aligned per its operating procedure, the CA system was operable at all times.

Corrective steps which have been taken and the results achieved:

An R&R was placed on valves in order to leave them in the "as found" position. The "as found" position was the desirable alignment for the current plant conditions.

Corrective steps which will be taken to avoid further violations:

a)

A representative from each shift will have this incident covered in a crew meeting. They will in turn be responsible for discussing proper use of operating procedures and R&R's with their respective shifts.
