SUBJECT: REPORT NOS. 50-321/86-28 AND 50-366/86-28
Thank you for your response of February 23, 1987, to our request for additional

information letter, dated January 12, 1987, concerning activities conducted at
your Plant Hatch facility.
The additional information was appreciated and
taken into consideration.
After careful consideration of the bases for your request that the violations

be classified as Severity Level V violations,-we have concluded, for the
reasons presented in the enclosure to this letter, that the violations should
.
remain as presented in the Notice of Violation.
We have evaluated your responses to the two violations and found that they meet
the requirements of 10 CFR 2.201. We will examine the implementation of your
corrective actions during future inspections.

ENCLOSURE
STAFF ASSESSMENT OF LICENSEE REQUEST
After evaluating the information provided, we have determined that the two
violations are properly classified as Severity Level IV violations for the
following reasons:
Violation A:
The control room ventilation surveillance procedure did not
contain instructions to ensure the technical specification
requirements were met.
Violation B:
The surveillance frequency specified in the technical
specifications was not met.
The Plant Hatch Surveillance
Coordinators' records are not considered as appropriate quality
. records for performance of the surveillance procedure on
October 25, 1982, and December 6, 1985.
Likewise, the undated
affidavit prepared and signed with no supporting data, as the
result of the inspectors' findings, to document the December 6,
1985, surveillance does not constitute a quality record.
We do not agree that you have shown that the violations had no
potential safety consequences.
The technical specification
requirements ensure that the system functions as analyzed to
meet the requirements of 10 CFR 50, Appendix A, Criteria 19.
The potential safety consequences associated with a failure to
meet 10 CFR 50, Appendix A, Criteria 19 requirements constitute
a violation which is of more than minor safety or environmental
significance.
