Gentlemen:

This refers .to the inspection conducted by Mr. .P. H. Harrell of this office

'during the period April 1-30, 1987, of activities authorized by NRC Operating

License DPR-40 for the Fort Calhoun Station, and to the discussion of our .

findings with Mr. W. G. Gates and other members of your staff at the conclusion

of the inspection.

Areas examined during the inspection included operational safety verification,

maintenance, surveillance, plant tours, safety-related system walkdown,

security observations, radiological protection observations, in-office review

of periodic and special reports, followup on previously identified items,.

followup on IE-Bulletins 86-02 and 86-03, refueling activities, containment

local leak rate testing, licensed operator initial training,-and licensed

operator requalification program. Within these areas, the inspection consisted

of selective examination of procedures and representative records, interviews

with personnel, and observations by the NRC inspector. The Inspection-findings

are documented in the enclosed inspection report.

During this inspection, it was found that certain of your activities were in .

violation of NRC requirements. Consequently, you are required to respond to

these violations, in writing, in accordance with the provisions of

Section 2.201 of the NRC's " Rules of Practice," Part 2, Title 10, Code of.

Federal Regulations. Your response should be based on the specifics' contained-

in the Notice of Violation enclosed with this letter.

During this inspection, it was also found that certain of your activities were

a potential violation of NRC requirements. However, since this potential

violation is related to training, it is not cited herein as a violation but is

considered to be an unresolved item in accordance with the Comission' Policy

Statement on Training and Qualification of Nuclear Power Plant Personnel

(50 FR 11147). We are concerned'that the individual SR0 was returned to

normal shift assignment subsequent to failing the annual written-

requalification examination. This matter needs specific management attention

to prevent this type of oversight in the future. No response is required; ^

however, your attention to the unresolved . item which has the potentic,l_ of

becoming a violation in the. future is warranted. !The unresolved item is

discussed in paragraph 15 of the enclosed inspection report.

'Also during this inspection, it was found that certain of your activities

appeared to deviate from a commitment made to the NRC. This item and reference

to the connitment is identified in the enclosed Notice of Deviation. You are

requested-to respond to this deviation in writing. Your response should be

based on the specifics contained in the Notice of Deviation enclosed with this

letter.

In addition to the unresolved item identified in paragraph 15 related to

training, three additional unresolved items are identified in paragraph 7 of

the enclosed inspection report. These three unresolved items regarding the

.

trisodium phosphate dodecahydrate (TSP) requirements need prompt attention.

We have also examined actions you have taken with regard to previously

identified-inspection findings. The status' of these items is identified in

paragraph 2 of the enclosed inspection report.

The responses directed by this letter and-the accompanying Notices are not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.
