3 86-012-0 HA INSTRU The Unit tripped from 90% RTP at 0020 hours2.314815e-4 days <br />0.00556 hours <br />3.306878e-5 weeks <br />7.61e-6 months <br /> EDT due to a spurious main turbine backup overspeed trip signal. The cause.for the over-speed trip signal could not be attributed to a specific component failure. Overspeed testing will be performed during unit startup. The Unit remained out of service at the end of the month. All major main-tenance items have been completed.
The Unit is estimated to be returned to service on June 7, 1986.

1 of 1 MONTHLY OPERATING ACTIVITIES - JUNE, 1986 HIGHLIGHTS:
The reporting period began with the. Reactor Coolant System in Mode 5 at 130*F.
The RCS was heated up.to Mode 4 and later cooled back down to Mode 5'due to Reactor Coolant Pump problems.
The reporting period ended with the-Reactor _ Coolant System in Mode 5 at 164*F.
SUMMARY
06-01-86 At 0652, operators started draining the RCS to half loop.
Half. loop was achieved at 0530 on 06-02-86.
06-12-86 At 2053, filling of the RCS began.
The fill was stopped at 0437 on 06-13-86.
06-13-86 At 0703, the pressurizer heatup began in preparation to draw a bubble.
At 1040 the bubble was established.
06-16-86 At 1948, the RCS entered' Mode 4.
06-17-86 At 0400, the RCS heatup was stopped.at 300*F.
. 23-86 At 1728, an RCS cooldown and depressurization to Mode 5 was' started because of reactor coolant pump seal and motor problems.
5 06-24-86 At 0200, the RCS entered Mode-5.
06-25-86 At 0620, operators started draining the RCS to half 4
loop.
Half loop was achieved at-0635 on 06-26-86.
06-29-86 At 0332, filling of the RCS began.
The fill'was-stopped at 0700.
At 0840, the pressurizer heatup began in preparation to 4
draw a bubble.
At 1350 the bubble was established.
06-30-86 At 2030, a depressurization of the RCS was begun for alignment of #11 reactor coolant pump.

50 - 315 UNIT NAME D. C. Cook - Unit No. 1 DATE 7-3-86 COMPLETED BY B. A. Svensson TELEPHONE (616) 465-5901 PAGE 1 of 1 MAJOR SAFETY-RELATED MAINTENANCE JUNE, 1986 M-1 CS-321, Containment Isolation Check Valve was leaking exces-sively. Valve was replaced and the new valve tested satis-factorily.
M-2 Reactor Coolant Pump No. 12 had excessive vibrations. Replaced lower motor guide shoe and aligned motor to pump and verified proper operation.
M-3 NRV-151, Pressurizer Power Operated Relief Valve leaked by.
Replaced stem, plug, and seat ring. Valve was retested and returned to service.
M-4 NRV-152, Pressurizer Power Operated Relief Valve was repaired due to leak-by indication. Replaced stem, plug, and seat' ring.
Valve retested satisfactorily.
M-5 NRV-153, Pressurizer Power Operated Relief Valve was repaired due to a leak-by problem. Replaced stem, plug, and seat ring after it was discovered that the old parts were steam cut.
The valve was retested satisfactorily.
C&I-l NIS Intermediate Range Channel, N-35, failed. The detector canister was found to have water in it.
Replaced the detector with a new detector and performed required calibration proce-dures.
