Dear Mr. Keppler:
An investigator employed by Pinkerton's, Inc. was assigned as a security guard at Quad-Cities Nuclear Power Station from March 21 through April 7,1977 The purpose of assigning the investigator was to determine if allegations by Mr. James E. McKay, a Pinkerton Guard, were credible.
Based on a recommendation by the investigator, Mr. McKay was offered another position by Pinkerton's Inc. and transferred from the station.
According to the investigator's reports, Mr. McKay was disliked by all of the station guards and by many of the Commonwealth Edison Company employes at the station.
It was the investigator's opinion that Mr. McKay was "a troublemaker and highly over-reactive and imagina tive".
The enclosure to the March 10 letter to Mr. Keppler included seven items which were to receive emphasis during the l
investigation.
The following paragraphs are keyed by number to the paragraph of the March 10 report and include the finding;s of the investigator:

The metal detector did not always work properly because it required sensitivity adjustments to detect keys and other small metal objects.
Appropriate adjustments are made as necest,ary and adjustment controls are secured by a lock.
Access to the adjustment controls is permitted by the shift supervisor.

Each time an employe attempted to exit the Protected Area j
with Coceany property without appropriate authorization, the employe was required to return to the plant for a property pass.
- 11. The guard filing cabinet was left unlocked at times, but only for several minutes and only if a guard was near by.
1 When the cabinet was not locked after use by a Suard, it was brought to the guards attention by a guard sergeant.

- 17. The guard test was found to be well written, difficult and thorough.
The investigator suggested some improvements for i
instructors which are being evaluated by Pinkerton's, Inc.
I The test is designed to cover the basic guard training, but not all specific station security procedures.
- 23. The investigator was unable to identify any guard at the station who has attempted to sell stolen materials.

- 24. The investigator did not observe any E.mploye depart without i
turning-in his security badge.
There have been no reports 1
of badges taken from the Protected Area since March lo,1977, when the guards were requested to include such occurrences in the guard report.

- 28. The investigator identified no basis for questioning any guard's integrity because of a large p.1rchase.

The alarms were installed and are provie ng adequate assurance that the doors are properly closed.
This letter addresses a subject matter exempt from disclosure according to Section 2 790 of the Nuclear Regulatory Commission " Rules of Practice", Part 2, Title 10, Code of Federal Regulations.

During the period March 2 through March 8, 1977, an investigation concerning allegations by Mr. McKay was conducted by the following individuals:
Each of the following paragraphs includes a statement of the allegation; a sumnary of findings; and, if appropriate, correct!.ve action taken:
1.

The metal detector does not always work properly.

The metal detector was checked and found to be operating proper The sensitivity of the metal detector is set to detec grams of ferrous metal.
In a telephone conversation with Mr. McKay on March 3, he explained that the ability of the guards to defeat the metal detector was as a result of repeated efforts and experimentation.
Since personnel normally entering the Protected Area are not provided with the opportunity to experiment with the metal detector, this is not considered to be a security weakness.

The Radiation Portal Monitor is not always working properly.
Two Radiation Portal Monitors are located in the guardhouse.
The monitors facilitate rapid egress from the site since the monitors nominally require ten seconds to complete a cycle.
Also, some redundancy is provided in the case of a failed component.
The monitors have experienced some failures, but not normally both at the same time.
If both monitors fail, a Geiger-Mueller counter is avail-able at all thnes at the guardhouse for radiation check-out purposes.
Radiation Chemis'try Technicians are on site at all times and are to be' called if the monitors fail.
During the day shift Monday through Friday, instrument mechanics are available to service the monitors.
The NRC recently investigated allegations concerning defective portal monitors and found no evidence of noncompliance.

Employes have been observed intentionally abusing dosimeters H
by throwing them against the window shade and floor of the guardhouse.
A Radiation Chemistry Technician acknowledged such abuse to be of a jovial nature after determining a dosi-meter to be inoperative and of no possible further use.
The purpose of the abuse was to render the dosimeters "obviously inoperable" so they could not be inadvertently reused.
Corrective Action:
The employe has been instructed to discontinue the practice of throwing dosimeters and an alternate method to insure that the dosimeters will not be inadvertently reused has been suggested.
6.
Unauthorized contractors have been observed in Vital Areas.
No positive action was taken when this was reported.
N In accordance with the Security Plan and Procedures, contractors are permitted unescorted access to Vital Areas as required in the performance of their duties.
There have been no reports of unauthorized contractor personnel i-in Vital Areas.

A guard was posted at the door on January 31 and February 1 while the door was being repaired.

Rolls of steel, scrap copper and sandpaper have been taken by employes without property passes.

It is suspected that theft of station property by employes and contractors does occur.
Theft of small N,
quantities of material is extremely difficult to detect.

Presently, no information is available which indicates that any specific employe or contractor is guilty of M"
theft of station property.

guards are knowledgeable of unreported thefts.

Employes frequently depart without turning in security I
badges.

occasionally an employe will depart the station and inadvertently fail to turn in this security badge.
Employes must return security badges to the guardhouse in order to

reenter the Protected Area.
This is not considered to be a frequent occurrence.
Corrective Action:
In order to determine frequency, guards
)have been instructed to include reports in the daily guard log of employes departing without turning in security badges.

- 12. Employes and contractors 'are frequently observed in hard hat areas without hard hats.
Enforcement of the OSHA hard hat requirement has at i
times been difficult, particularly with contractor Y
personnel, however, it does not present a security problem.
Recent progress has been made with both station and
'l construction personnel since increased emphasis has been placed on wearing hard hats.
Contract specifications contain a statement referencing the hard hat requirement and its implementation.

- 13. Floor plans of the plant from the guard procedure are

i The exact nature of the drawings which may be missing from the guard procedure cannot be determined.
j At some time in the past, drawings may have been available l i/
to the guards which showed the Protected Area, perimeter and gate numbers, or engineering drawings which showed the plant floor plan.
Drawings of this type have been widely distributed and if such drawings are missing from
.i the guard procedure, the loss of the drawings does not have a significant adverse affect on the station security program.
A set of engineering drawings showing the plant floor plan were located in a desk drawer in the guardhouse.

Fish impingement is monitored at the station pursuant to the station's Technical Specifications.
Technical Specification Appendix B.2.3.A requires that an inventory

of fish entrappment on the intake structure be made twice por week.
This information is reported to the NRC in a semi-annual report, " Operation Environmental Monitoring in the Mississippi River near Quad-Cities Station" by Nalco Environmentai Sciences.
Additionally, the information is reported to the Illinois and Federal Environmental l
Protection Agencies as a requirement of the Station's NPDES pennit.

- 19. A vehicle was not provided for required guard patrols in extremely cold weather.
Guard patrols are conducted in accordance with the Security Plan and Procedures regardless of the weather conditions.
Approximately three months ago an electric car purchased for use by the security force was returned to the vendor for repair.
The car was experimental in nature and the malfunction could not be diagnosed or: site.
The vendor did not progress in the repair of the vehicle, therefore it was retrieved during the week of February 28 through March 4 and repairs are to be undertaken by Commonwealth Edison.
- 20. The Owner Controlled Area gate # 28 is broken.
Gate # 28, an Owner Controlled Area gate, which is not in the Protected Area barrier, was damaged.
A work request to repair the damage was submitted on November 19, 1976 and resubmittal on January 12 and January 26, 1977.
The gate is scheduled for repair on March 9.
- 21. Vital Area doors are frequer.tly found intentionally left open.

Corrective Action:
Station management is evaluating e use local audible alarms f or installation on the Vital Area doors.
These alarms would insure personnel awareness of a door which was not properly closed.

There is not a security requirement to lock the administrative area doors.
The unlocked administrative areas are not Vital Areas.

The number of employes departing the station at lunch is quite small since only 30 minutes is allowed; 1
therefore, there is no reason to believe that employes i
slip out unaccounted for.
It is the practice of some guards not to file the secur.ity badges for the employes departing for lunch in their respective slots since they return after a very short period of time.
This item will, however, be included in the investigation by Pinkerton's, Inc.
- 25. The electric car for the guards was abused and has not i
I' been returned for the use of the guards.

The status of repairs to the car is provided by Item 19.

- 26. Badge photos: some were expired and not replaced; one guard was photographed in civilian clothes.
The Station Security Office maintains an effective suspense and notification system for insuring that badges are reissued prior to expiration dates.
A monthly list of badges which will expire is provided to the guards.
The guardssverbabl'y notify employes of expiration and if necessary attach notes to the badges.
If a badge is not renewed prior to the expiration date it is removed from the badge rack and provided to the Station Security Officer.
At times a guard photograph on a security badge may be made with the guard dressed in civilian clothes.
This lack of uniformity in badge photographs for guards has not adversely affected station security, however, in the future guard photographs for badges will be taken with the guard wearing a uniform.
