Dear Mr. Gagliardo:
Inspection Report 86-16 Notice of Violation Omaha Public Power District (0 PPD) recently received Reference 2, containing a notice of violation.
The violation involved acceptance of vendor material whose material test report did not certify to all tests required by the procurement specification.
CPPD's response to this violation is attached to this letter.

ATTACHMENT During an NRC inspection conducted on May 27-29, 1986, a violation of NRC requirements was identified. The violation involved acceptance of vendor material whose material test report did not certify to all tests required by the procurement specification.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed below:
10 CFR Part 50, Appendix B, Criterion VII, states, in part, " Measures shall be established to assure that purchased material... conform to the procure-ment documents...
Documentary evidence that material and equipment conform to the procurement requirements shall be available at the nuclear power plant... and shall be sufficient to identify the specific requirements
... met by the purchased material...."
Contrary to the above, measures were not established to prevent the acceptance by the licensee of electrical cable purchased to P.O. 526016, whose material test report was not sufficient to ensure that all procurement specification requirements were met.
This is a Severity Level V violation.

OPPD RESPONSE 1.
The Reason for the Violation. If Admitted The violation cited occurred as a result of the lack of documentation to demonstrate a clear auditable trail from the vendor Certificate of Compli-ance to the Qualification Test Report.
The Rockbestos cable purchased per P.O. 526016 was certified by Rockbestos tomeetSpec.No.gSEE0504Rev.2.
GSEE-0504 Rev. 2 requir'ad cable quali-fication to 5 x 10 rads (Part 3.04). One anomaly noted is that the certificate of conformance does not specify that the certified item is Rockbestos Firewall III. However, the product code listed on the certifi-cate of conformance does list a lot code which is identifiable as Firewall III. OPPD has a report on file qualifying Rockbestos Firewall III cable for high radiation areas. The cable in stores is clearly marked Firewall III.
The data above indicates that OPPD was justified in accepting this cable and did properly control the purchased cable.

Corrective Steos Which Have Been Taken and the Results Achieved Appropriate documentation has been added to the Purchase Order file to correct the omission of Firewall III on the certificate of conformance.

The Corrective Steos Which Will Be Taken to Avoid Further Violations In the future, OPPD will attempt to ensure that Firewall III is noted on the certificate of conformance. However, OPPD believes that the omission of this notation would not disqualify a cable from use in safety related systems as long as the cable is qualified to GSEE-0504. The purchasing specifications are currently undergoing revision which will require certi-fication to a cable qualification test report. This will preclude further omissions of this sort.
