' APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company Docket No. 50-353 Limerick Unit 2 License No. CPPR-107 As a result of the inspection conducted on_ September _29, l'986 to November 2, 1986 and.in accordance with 10 CFR 2, Appendix C,.the following violation was identified.
'A. 10 CFR 50, Appendix B Criterion XI and the Limerick Preliminary Safety Analysis Report Appendix 0.4.3 require that construction hydrotests be performed in accordance with written test procedures to demonstrate that the piping system will' perform satisfactorily in service. ASME Boiler and Pressure Vessel' Code Section III ND-6215 and Bechtel Specification LP-323 require that all piping be thoroughly examined for leaks during hydrotests.
' Contrary to the above, on October 16, hydrotest 2M-83A-44 on the Main Steam relief piping was improperly performed as Field Weld 3 was not visually examined by quality control or-field engineering during the hydrotest.
This is a_ Security Level V Violation (Supplement II).
Based upon the corrective actions implemented by Philadelphia Electric Company as documented in section 6d of this report, no response is
-required for this violation.
