This refers to the inspection conducted by Mr. E. Yachimiak of this office on October 7-10, 1986, of your facility in Parshalltown, Iowa and to the discussions of our findings with Mr. D. Rowley and members of your staff at the conclusion of the inspection.
This inspection was made as a result of an interim 10 CFR 50.55(e) deficiency report from Vogtle nuclear electric generating plant. The report was made due to the failure of a number of manually operated butterfly valve actuator gears. Areas examined during the inspection and our findings are discussed in the enclosed report. Within these areas, the inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspector.
During the inspection it was found that the implementation of your QA program failed to meet certain NPC requirements. Violations to 10 CFR Part 21.21 ard Part 21.31 were noted with regard to non-ASME nuclear parts and components.
Also, two nonconformances to Appendix B to 10 CFR Part 50 were identified, one in the area of receipt inspection and the other in the area of corrective action.
l The corrective action deficiency is of concern because notification to your l nuclear customers of changes in manual valve actuator maximum torque ratinas may have prevented actuator gear failures at their facilities. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.
The enclosed Notice of Violation is sent to you pursuant to the provisions of Section 206 of the Energy Reorganization Act of 1974. You are required to submit to this office witMn 30 days from the date of this letter a written statement containing: (1) a description of steps that have been or will be taken to correct these items; (2) a description of steps that have been or will be taken to prevent recurrence; and (31 the dates your corrective acticns and preventive measures were or will be completed. Consideration may be given to extending you: response time for good cause shown.
