9 ENCLOSURE 1 TO SBN-1235 NOTICE OF VIOLATION

' Notice of Violation 86-46-01 10CFR50, Appendix B, Criterion XVI and -the Seabrook Station FSAR, Section 17.1.1.16 require that measures be established to assure conditions adverse to quality and nonconformance be promptly identified and corrected. Regulatory Guide 1.29 and Seabrook Station FSAR Section 1.8 specify requirements for non-safety items, the failure of which could affect safety-related equipment.

Specifically, seismic design and construction is required of non-saf tey items whose failure could reduce the functioning of safety-related plant. features.

New Hampshire Yankee (NHY) procedure ASP-5 specifies bailding turnover acceptance criteria, to ' include the removal of temporary construction items.

Contrary to the above, as of August 18, 1986, non-safety-related components were located'within the service water cooling tower in such a configuration that failure of such equipment could damage safety-related piping and valves of the service water system. These items were temporary construction items, not seismically designed or constructed, and yet were not identified on any incomplete items list or work control of the station staff, that licensee programmatic measures (e.g., NHY procedure ASP-5 controls) had failed to provide for' prompt identification and correction of such conditions adverse to q uality.

Response

The-violation occurred due to a failure to comply with " Building / Area Turnover

. Acceptance Criteria" provided in NHY Procedure ASP-5.

Corrective Action Immediately af ter the finding, Project Management was notified and the temporary items were removed. Quality Assurance Surveillance checklists were revised and additional housekeeping surveillances were performed by Quality Control.

Corrective Action to Prevent Recurrence A.

The Quality Assurance Surveillance Checklist has been revised to include an attachment for honaekeeping walkdowns with provisions to identify temporary items.

B.

New Hampshire Yankee Procedure MA3.1, Revision 6, Titled " Work Requests" has been ' revised to include provisions for a cleanliness inspection of the work area prior to the close of Work Requests. Training on the implementation of this procedure is presently being conducted.

Construction management has issued a Construction Notice, instructing

. personnel to tag temporary items and. identify Work Request numbers on the

' tag. The notice also instructs personnel to remove temporary items upon completion of the work.

D.

New Hampshire Yankee Procedure MA 3.3, titled " Housekeeping", has been revised to include. provisions for periodic walkdowns to be administered by the Radwaste/ Utilities Department Supervisor. These walkdowns will identify temporary items, not specifically associated with Work Requests or Building Incomplete Items Lists. To expedite the prompt identification and correction of this condition, the above referenced walkdowns frequency will be increased to facilitate a scheduled completica date of December 31, 1986.
