L DUKE POWER COMPANY McGUIRE NUCLEAR STATION VIOLATION RESPONSE Violation'50-369/87-08-01 Technical Specification 6.8.1 requires that written approved procedures be established, implemented and maintained covering the operation and maintenance of safety-related plant equipment.
Station Directive 4.4-2 Control of Temporary Modifications defines a lifted lead as one of fourteen types of temporary _ modifications and delineates the administrative controls designed to maintain management of plant equipment.
Activities directly related to an active ongoing test procedure may be excluded from the requirements of directive 4.4.2 if the controls embodied in the test procedure are equal to those of the directive..

Unit 1 Refueling Water Storage Tank heater control rendering the circuit j
inoperable without employing the requirements of. Station Directive 4.4.2 or an

active test procedure.

This is a Severity Level IV Violation (Supplement I) and is applicable to-Unit 1 only.

RESPONSE
1.
Admission or denial of violation:
Duke admits the violation occurred as stated.
2.
Reason for the violation if admitted:
Violation was due to a misunderstanding regarding the testing of NS heat exchanger on Unit 1.
Shift personnel understood the FWST heaters were needed to raise temperature of FWST to perform a heat exchanger efficiency test on NS.
The procedure used for the NS heat exchanger test

covered the removal of the lead in question. Under the' assumption that this test, PT/1/A/4208/04B, was to be performed, the shift crew lifted the lead and documented this in procedure, IP/0/A/3090/02. However, since the PT on NS heat exchanger was not to be performed on Unit 1, the lif ted lead should have been documented as a Temporary Modification under Station Directive 4.4.2.
3.
Corrective steps which have been taken and the results achieved:
Corrective action included a discussion with all Shift Engineers to insure that before work is performed that the test log be checked to verify that identified testing is in progress.
In addition the Operating-Superintendent issued instructions to all shift personnal stating that clearance will not be given to perform work on Technical Specification related items unless the work is identified on the Operating Schedule.
i This will insure that correct information is used to identify testing scheduled to be performed.
